摘要
目的 分析老年高血压患者的临床特点,研究合理的降压策略。方法 收集于潍坊市中医院心血管内科就诊的40岁以上高血压患者一般资料,按年龄分为中年组及老年组,并将老年组高血压患者随机分为强化降压组和标准降压组,随机分组后第1,3,6个月各随访1次,此后每6个月随访1次,直至研究结束。结果 老年组高血压患者收缩压、脉压明显高于中年组(均P<0.05),舒张压明显低于中年组(P<0.05),高血压知晓率、控制率均显著低于中年组(均P<0.05)。老年高血压患者强化降压组非致死性急性心肌梗死、卒中和心脑血管死亡率明显低于标准降压组(均P<0.05)。结论 老年高血压患者以收缩压升高为主,脉压差大,知晓率、控制率明显低于中年组,强化降压可明显降低非致死性急性心肌梗死、卒中和心脑血管死亡率。
Objective To study the appropriate antihypertensive strategies by analyzing the clinical characteristics of elderly patients with hypertension.Methods The general data of hypertension patients over 40-years-old in the department of cardiovascular medicine, Weifang Hospital of Traditional Chinese Medicine were collected and divided into middle-aged group and elderly group.The elderly hypertensive patients were randomly divided into intensive antihypertensive group and standard antihypertensive group.The patients were followed up in the first, third, sixth month after randomization, and every 6 months thereafter until the end of the study.Results Systolic blood pressure and pulse pressure in the elderly group were significantly higher than those in the middle-aged group(all P<0.05).The diastolic blood pressure was significantly lower than that in the middle aged group(P<0.05).The awareness rate and control rate of hypertension were significantly lower than those in the middle aged group(P<0.05).The rate of non-fatal acute myocardial infarction, stroke and cardiovascular and cerebrovascular mortality in the intensive antihypertensive group were significantly lower than those in the standard antihypertensive group(all P<0.05). Conclusion The elderly patients with hypertension are mainly with elevated systolic blood pressure, and large pulse pressure difference, the awareness rate and control rate are significantly lower than the middle-aged group.Intensive antihypertensive therapy can significantly reduce the non-fatal acute myocardial infarction, stroke and cardiovascular and cerebrovascular mortality.
