摘要
目的:观察内镜套扎术治疗食管胃底静脉曲张破裂出血(EGVB)的疗效。方法:回顾性选取2017年6月至2021年12月在泉州市急救指挥中心接受内镜套扎术治疗的74例EGVB患者的资料,37例接受单纯内镜套扎术(EVL)治疗的患者纳入对照组,将其中37例接受EVL联合硬化剂(EIS)、组织胶(TAI)注射治疗的患者纳入观察组。比较两组患者术后72 h内的疼痛程度评分,出院前统计两组患者术后并发症的发生情况,术后1个月评估两组患者的手术疗效,术后3个月比较两组患者手术效果相关指标:曲张静脉数、曲张静脉直径、门静脉血流量的变化。结果:观察组患者术后24 h、48 h、72 h的疼痛程度评分均低于对照组,差异具有统计学意义(P<0.05);观察组临床疗效优于对照组,差异具有统计学意义(Z=-2.787,P<0.05);术后3个月,两组患者曲张静脉数、曲张静脉直径、门静脉血流量均低于术前,且观察组均低于对照组,差异具有统计学意义(P<0.05)。结论:在单纯EVL的基础上联合EIS、TAI注射治疗能够进一步提高临床疗效,且手术方案安全可行。
Objective To observe the efficacy of endoscopic ligation in the treatment of esophageal and gastric varices bleeding (EGVB).Methods The data of 74 patients with EGVB treated by endoscopic ligation in Quanzhou City Emergency Command Center from June 2017 to December 2021 were retrospectively selected.Among them,37 patients treated by endoscopic variceal ligation (EVL) alone were included in the control group.The other 37 patients who received EVL combined with endoscopic injection sclerosis (EIS) and tissue adhesives (TAI) injection were included in the observation group.The changes of pain degree within 72 hours after operation were compared between the two groups,and the occurrence of postoperative complications were statistically analyzed before discharge.The efficacy of the two groups was evaluated at 1 month after operation.Three months after operation,the indexes related to the operation effect of the two groups were observed:the number of varicose veins,the diameter of varicose veins,and the changes of portal vein blood flow.Results The postoperative pain score at 24 h,48 h and 72 h of the observation group were lower than those of the control group.The clinical efficacy of the observation group was better than that of the control group.Three months after operation,the number of varicose veins,varicose vein diameter and portal blood flow in the observation group were lower than those in the control group,and the differences were statistically significant (P < 0.05).Conclusion Combining with EIS and TAI injection based on endoscopic ligation alone can further improve the clinical efficacy,and the surgical plan is safe and feasible.
