摘要
帕金森病(Parkinson’s disease, PD)是仅次于阿尔茨海默病的第二大中枢神经退行性疾病。目前认为,PD的发病机制主要是氧化应激、α-突触核蛋白聚集、神经炎性反应及线粒体功能障碍等导致的中脑黑质区DA能神经元的丢失、死亡,进而诱导出PD表型。然而,以上机制并不能完全解释PD的病程长、致残率高、临床治疗效果不佳等问题。因此,努力探究PD发病的新机制和治疗靶点是PD研究工作的重点。近年来,随着对PD病理研究的不断深入,人们逐渐认识到PD的病理过程中存在一种新型的细胞死亡形式,即铁死亡。研究发现,PD患者脑组织中存在铁代谢紊乱、脂质过氧化、活性氧自由基增多等与铁死亡发生密切相关的病理变化,且中医药(traditional Chinese medicine, TCM)和针灸治疗对纠正这些铁死亡相关因子和表达水平的失衡有着显著的作用。因此,阐明TCM和针灸治疗通过抑制铁死亡改善患者PD症状的新机制显得尤为重要。对铁死亡及其在PD病理中的作用和近年来有关通过TCM及针灸治疗PD的作用机制及疗效进行了综述。
Parkinson’s disease(PD) is the second most common neurodegenerative disease after Alzheimer’s disease. Currently, the pathogenesis of PD is mainly associated with oxidative stress(OS),α-synuclein aggregation, neuroinflammation and mitochondrial dysfunction, leading to the loss and death of dopaminergic neurons in the substantia nigra of the midbrain and then inducing the phenotype of PD. However, the above mechanisms could not fully explain the long course, high disability rate and poor clinical treatment effects of PD. Therefore, the efforts are taken to explore the new pathogenesis and therapeutic targets for PD. In recent years, with the deepening of PD pathological research, people gradually realize that there is a new form of cell death in the pathological process of PD,that is ferroptosis, which plays an important role in the pathological process of PD. It is evidenced that there are pathological changes closely related to the occurrence of ferroptosis in the brain of PD patients, such as iron metabolism disorder, lipid peroxidation and increase of reactive oxygen free radicals, and the treatment with traditional Chinese medicine(TCM) and acupuncture corrected the above imbalances through the expressions of related factors. Therefore, it is particularly important to clarify the new mechanisms of TCM and acupuncture to improve PD symptoms by inhibiting ferroptosis. This paper reviews ferroptosis and its role in PD pathology, as well as the mechanism and efficacy of TCM and acupuncture in PD treatment during recent years.
