摘要
目的探讨TrueBeam与Axesse系统对乳腺癌保乳术后切线弧容积旋转调强放疗(t-VMAT)计划的剂量学特性,评估其计划质量与剂量学质量控制检验结果,为临床应用提供参考。方法选取2021年8月至2022年2月在福建省肿瘤医院接受放射治疗的16例保乳手术后左侧乳腺癌女性患者为研究对象。对16例患者均利用TrueBeam加速器的Eclipse系统和Axesse加速器的Monaco系统分别设计t-VMAT计划,比较两套系统设计的计划靶区、危及器官剂量学差异,并利用旋转照射剂量测量仪ArcCHECK进行计划剂量学质量控制检验,比较其剂量验证Gamma通过率。结果两套系统设计的乳腺癌计划均能满足临床要求;靶区方面,均能达到大于95%的处方剂量覆盖;危及器官方面,Eclipse计划的脊髓、心脏、肺受量更低,Monaco计划的健侧乳腺受量更低;跳数方面,Eclipse计划的平均机器跳数比Monaco计划少43.9%;剂量学质量控制方面,两套系统在3%/2 mm标准下的计划验证Gamma通过率都大于95%,均达到临床质量控制要求。结论对于乳腺癌t-VMAT计划,两套系统均能满足临床应用需求,Eclipse计划对脊髓、心脏、肺受量的保护更优,Monaco计划对健侧乳腺的保护更优,需根据临床实际情况选择合适的放疗系统。
Objective To study the dosimetric characteristics of tangential volumetric modulated arc therapy(t-VMAT)plans for breast cancer radiotherapy between TrueBeam and Axesse systems,and to assess their planning quality and quality control results for providing references to clinic use.Methods A total of 16 patients with left breast cancer after breast-conserving surgery were selected,and t-VMAT plans were designed using the Eclipse system of the TrueBeam accelerator and the Monaco system of the Axesse accelerator,respectively.Dosimetry differences in tumor target and organs at risk were compared.Meanwhile,dosimetry quality control(i.e.dose verification)was performed using ArcCHECK to compare the Gamma pass rates.Results The breast cancer plans designed by the two sets of radiotherapy systems can meet the clinical requirements.For tumor targets,both system can achieve greater than 95%coverage of the prescribed dose;for organs at risk,the Eclipse plans are lower in the doses of the spinal cord,heart,and lung,while the Monaco plans are lower in the doses of the contralateral breast;for the number of monitor units,the Eclipse plans are 43.9%less than the Monaco plans;for dosimetry quality control,the Gamma pass rates of plan verification of the two systems are greater than 95%under the standard threshold of 3%/2mm,and both meet the clinical quality control demands.Conclusions For the t-VMAT planings of breast cancer,both systems can meet the needs of clinical applications.The Eclipse plans protect the spinal cord,heart and lung better,and the Monaco plans protect the contralateral breast better.It needs to choose the appropriate radiotherapy system according to the actual clinical situation.
