细胞学和HPV基因分型对宫颈癌风险分层及HPV阳性妇女的诊断价值 被引量:2

摘要 目的通过联合HPV16、HPV18和其他高危基因型,以及宫颈癌筛查人群中女性的细胞学结果来评估CIN3+和CIN2+的风险。方法2018年3月至2021年3月,共有500例33~68岁的女性在基线检查时HPV检测呈阳性,并接受了细胞学检测。根据细胞学结果和HPV基因型(HPV16、HPV18和其他高危HPV),对3级或更严重的宫颈上皮瘤变(CIN3+)进行95%置信区间的风险估计。结果所有HPV阳性患者中,患高级别CIN2+的达78.8%(95%CI 77.9~80.3),患高级别CIN3+的达68.2%(95%CI 67.3~69.7);HPV16患高级别CIN2+的达84.74%(95%CI 80.12~89.57),患高级别CIN3+的达74.1%(95%CI 69.5~79.0);HPV18患高级别CIN2+的达71.39%(95%CI 60.32~79.97),患高级别CIN3+的达60.8%(95%CI 49.7~69.4);其他高危HPV风险中,患高级别CIN2+的达68.4%(95%CI 65.9~71.9),患高级别CIN3+的达57.8%(95%CI 55.3~61.3)。结论通过在筛查中纳入HPV基因分型,并与HPV16/18相比,对其他高危HPV选择更保守的治疗方法,可以更好地为女性宫颈癌防治提供依据。 Objective To evaluate the risk of CIN3+and Cin2+by combining HPV16,HPV18 and other high-risk genotypes and cytological results of women in cervical cancer screening population.Methods From March 2018 to March 2021,a total of 500 women aged 33 to 68 were HPV positive at baseline and underwent cytology.Based on cytological results and HPV genotypes(HPV16,HPV18 and other high-risk HPVs),the risk of grade 3 or more cervical epithelial neoplasia(CIN3+)was estimated by 95%confidence interval.Results Among all HPV positive patients,78.8% had high-grade Cin2+(95%CI 77.9-80.3),68.2%(95%CI 67.3-69.7)suffered from high-level CIN3+;84.74%(95%CI 80.12-89.57)of HPV16 patients with high-level Cin2+and 74.1%(95%CI 69.5-79.0)of HPV16 patients with high-level CIN3+;71.39%(95%CI 60.32-79.97)of HPV18 patients with high-level Cin2+and 60.8%(95%CI 49.7-69.4)of HPV18 patients with high-level CIN3+;Among other high-risk HPV risks,68.4%(95%CI 65.9-71.9)had high-level Cin2+,and 57.8%(95%CI 55.3-61.3)had high-level CIN3+.Conclusion By including HPV genotyping in screening and selecting more conservative treatment methods for other high-risk HPV compared with HPV16/18,we can better provide basis for the prevention and treatment of female uterine cancer.
