摘要
[目的]探讨磁控胶囊内镜(MCCE)检查中卧位或立位吞服胶囊内镜对食管的观察效果。[方法]前瞻性分析行MCCE检查的270例的临床资料,观察比较卧位与立位吞服胶囊内镜时,齿状线可视范围、MCCE通过食管时间、食管病变检出率及安全性。[结果]卧位吞服组齿状线可视度明显高于立位吞服组(64.4%∶38.6%,P<0.001);卧位吞服组的MCCE通过食管的时间明显长于立位吞服组[(84.1±6.14)s∶(17.01±4.73)s,P<0.001];卧位吞服组病变检出率高于立位吞服组(26.0%∶13.3%,P<0.001)。所有检查者均无胶囊滞留。[结论]卧位吞服胶囊内镜组比立位组对食管观察更清楚,有利于提高食管病变检出率。
[Objective]To investigate the observation ability of swallowing capsule endoscopy in decubitus or upright position during magnetic controlled capsule endoscopy(MCCE)examination.[Methods]Clinical data of 270 people undergoing MCCE in the digestive endoscopy center of wuhan union medical college hospital were prospectively analyzed.Statistical analysis was made on time for MCCE to pass through esophagus, dentate line visibility, esophageal lesion detection rate and safety under capsule endoscopy in recumbent position and upright position.Comparison was made on the diagnostic value and safety of capsule endoscopy in the two positions for esophageal lesions.[Results]The dentate line detection rate was significantly higher in the recumbent position group than in the upright position group(64.4% vs 38.6%,P< 0.001).The time for MCCE to pass through the esophagus was significantly higher in the recumbent position group than in the upright position group([84.1±6.14]s vs[17.01±4.73]s, P< 0.001).The positive lesion detection rate was superior in the recumbent position group compared with the upright position group(26.0% vs 13.3%,P<0.001).All examinees had no capsule retention.[Conclusion]The recumbent position group has higher observation ability of esophagus than the upright position group, which is beneficial to improve the detection rate of esophageal lesions.
