内牵引技术在腹膜后机器人辅助肾腹侧肿瘤部分切除术中的应用效果 被引量:1

摘要 目的观察内牵引技术在腹膜后机器人辅助肾腹侧肿瘤部分切除术中的安全性和有效性。方法回顾性分析2019年10月至2021年6月青岛大学附属烟台毓璜顶医院32例腹侧肾肿瘤患者应用内牵引技术行腹膜后机器人辅助肾部分切除术的资料。分析32例患者的手术时间、术中出血量、热缺血时间、手术切缘、术后住院时间、R.E.N.A.L评分、肾功能指标及术后并发症等。结果所有患者均顺利完成手术,无术中转开放或改根治性肾切除。中位R.E.N.A.L评分为8分(范围5~10分),术中平均热缺血时间(17.06±2.87)min,术中失血量(128.84±43.69)ml,手术时间(112.84±12.00)min,中位术后住院天数为3 d。无腹腔脏器损伤、感染及继发出血等并发症。病理结果:透明细胞癌29例,乳头状癌3例。无切缘阳性病例,中位随访6个月无局部复发或远处转移。结论对于腹侧肾肿瘤,利用内牵引技术行腹膜后机器人辅助肾腹侧肿瘤部分切除术安全有效。 Objective To explore the safety and effectiveness of internal traction technology in retroperitoneal robot-assisted partial nephrectomy for ventral renal tumors.Methods From October 2019 to June 2021,a total of 32 patients with ventral renaltumors underwent retroperitoneal robot-assisted partial nephrectomy.The surgical out comes with internal traction technology were analyzed retrospectively.The operative time,intraoperative bleeding,warm ischemia time,surgical margin,hospitalization time,R.E.N.A.L score,renal function index,and postoperative complications of 32 patients were analyzed.Results All the patients completed the operation successfully,and there were no conversions to open or radical nephrectomy.The median R.E.N.A.L score was 8(range,5-10).The average warm ischemia time during the operation was(17.06±2.87)min,the intraoperative bleeding volume was(128.84±43.69)ml,the operative time was(112.84±12.00)min,and the median postoperative hospital stay was 3 days.There were no complications,such as abdominal organ injury,infection,and secondary bleeding.The pathologic results were clear cell carcinoma(n=29)and papillary carcinoma(n=3).All patients had negative margins,and no local recurrences or distant metastases occurred during a median follow-up of 6 months.Conclusion Retroperitoneal robot-assisted partial nephrectomy for ventral renal tumors using internal traction technology is safe and effective.
