摘要
目的为儿童临床合理使用抗肿瘤药物提供参考。方法选取某院2017年1月至2019年12月上报至中国医院药物警戒系统的儿童使用抗肿瘤药物引发的药品不良反应(ADR)报告145例,统计患儿的性别、年龄、原患疾病、用药种类、给药途径、ADR发生时间、累及系统/器官及临床表现、ADR类别及转归情况并分析。结果145例ADR报告中,男性患儿(62.07%)多于女性(37.93%);年龄以小于6岁最多(82例,56.55%);原患疾病以白血病最多(140例,96.55%),其中又以急性淋巴细胞白血病较多(126例,86.90%);ADR涉及最多的药物类别为抗代谢类(147例次,52.88%);给药途径主要为静脉注射(180例,64.75%),尤以静脉滴注较常见(145例次,52.16%);ADR多发生于用药3 d内(59例,40.69%);累及系统/器官主要为血液系统(110例次,59.78%),主要临床表现为骨髓抑制(107例次,58.15%)。结论临床应加强患儿抗肿瘤药物ADR的监测与上报,了解ADR的特点及规律,做好预防工作,为儿童临床安全、合理、个性化用药提供参考。
Objective To provide a reference for the rational use of anti-tumor drugs in children.Methods A total of 145 children with adverse drug reactions(ADR)induced by anti-tumor drugs reported by a hospital to the China Hospital Pharmacovigilance System(CHPS)from January 2017 to December 2019 were selected,and their gender,age,primary disease,categories of used drugs,routes of administration,organs/systems involved in ADR,clinical manifestations,calssification of ADR and outcomes were analyzed.Results Among 145 ADR reports,boys(62.07%)were more than girls(37.93%),the children's age was mainly less than six years old(82 cases,56.55%),leukemia(140 cases,96.55%)was the most common disease,among which acute lymphoblastic leukemia(126 cases,86.90%)was the most common.The anti-tumor drugs with the highest incidence of ADR were anti-metabolites(147 cases,52.88%).The main route of administration was intravenous injection(180 cases,64.75%),especially intravenous drip(145 cases,52.16%).ADR mostly occurred within 3 d after drug administration(59 cases,40.69%).The system/organ involved in ADR was mainly the blood system(110 times,59.78%),and the main clinical manifestation was myelosuppression(107 times,58.15%).Conclusion ADR monitoring and reporting of anti-tumor drugs for children should be strengthened in the clinic,the characteristics and rules of ADR should be understood,and the prevention work should be done well,so as to provide a reference for clinical safe,reasonable,and individual medication in children.
