摘要
糖尿病是全球范围内发病率较高的疾病,虽然近年来得到了有效的控制,但在我国,糖尿病发病率仍呈现逐年增长的趋势。目前,我国已发展成为全球22个结核病高发病率的国家之一。大量流行病学研究证实糖尿病与结核病发病密切相关,经部分地区筛查发现结核病中糖尿病患者占比突出。免疫功能损害、营养不良、代谢紊乱等是并发肺结核的主要原因,其病情进展迅速、症状严重且治疗效果不甚理想,严重影响患者日常生活和生存质量。故在并发患者中进行双向筛选对于早期诊断、治疗及改善预后中具有重要意义。本文就近年来对于糖尿病合并肺结核疾病的发病机制及临床治疗等研究入手,着重对其中的免疫学及炎症机制及诊疗方面进行综述报道。
Diabetes is a disease with a high incidence rate in the world.Although it has been effectively controlled in recent years,the incidence rate of diabetes is still increasing year by year in China.At present,China has developed into one of the 22 countries with high incidence rate of tuberculosis in the world.A large number of epidemiological studies have confirmed that diabetes is closely related to the incidence of tuberculosis.Screening in some areas has found that diabetes accounts for a prominent proportion of tuberculosis.Immune function impairment,malnutrition,metabolic disorders are the main causes of complication of pulmonary tuberculosis,which has rapid disease progression,serious symptoms,and undesirable treatment effect,and seriously affects the daily life and quality of life of patients.Therefore,two-way screening in patients of diabetes complicated with pulmonary tuberculosis is of great significance for early diagnosis,treatment and improvement of prognosis.This paper reviews the research on the pathogenesis and clinical treatment of diabetes complicated with pulmonary tuberculosis in recent years,focusing on the immunology,inflammatory mechanism,diagnosis and treatment.
