摘要
目的分析中药穴位贴敷联合腕踝针对股骨粗隆间骨折患者术后疼痛的影响。目的选取厦门市中医院2019年7月—2020年7月股骨粗隆间骨折患者80例,均行股骨近端髓内钉内固定术,其中40例术后行常规护理干预为对照组,另40例在对照组基础上给予中药穴位贴敷联合腕踝针为观察组,比较2组术后不同时段疼痛程度(VAS)、爆发痛次数、止痛剂用量、睡眠时间、护理满意度、不良反应发生率。结果观察组术后12 h、1 d、3 d、7 d VAS评分均低于对照组(P<0.05);观察组爆发痛次数、止痛剂用量较对照组少,睡眠时间较对照组长(P<0.05);观察组护理满意度95.00%(38/40)较对照组77.50%(31/40)高(P<0.05);观察组术后不良反应发生率7.50%(3/40)低于对照组25.00%(10/40)(P<0.05)。结论中药穴位贴敷联合腕踝针应用于股骨粗隆间骨折患者术后镇痛护理,可缓解术后疼痛程度,预防爆发痛,减少止痛剂用量,延长睡眠时间,提高护理满意度,安全可靠。
Objective To analyze the effect of acupoint application of Chinese medicine combined with wrist and ankle needle on postoperative pain in patients with intertrochanteric fracture.Methods From July 2019 to July 2020,80 cases of femoral intertrochanteric fracture patients received proximal femoral intramedullary nail fixation were selected.40 cases of postoperative routine nursing intervention were selected as control group,and another 40 cases received acupoint application of Chinese medicine combined with wrist and ankle needle on the basis of the control group as the observation group.The pain scale(VAS),times of pain outbreak,dosage of analgesics,sleep time,nursing satisfaction and incidence of adverse reactions were compared between the two groups at different time periods after operation.Results The VAS scores at 12 h,1 d,3 d and 7 d after operation in the observation group were lower than those in the control group(P<0.05).The frequency of pain outbreak and analgesic dosage in the observation group were less than those in the control group,and the sleep time was longer than that in the control group(P<0.05).The nursing satisfaction of the observation group was 95.00%(38/40)higher than that of the control group 77.50%(31/40)(P<0.05);The incidence of postoperative adverse reactions in the observation group was 7.50%(3/40)lower than that in the control group 25.00%(10/40)(P<0.05).Conclusion The acupoint application of Chinese medicine combined with wrist and ankle acupuncture in postoperative analgesic nursing of patients with intertrochanteric fracture can alleviate postoperative pain,prevent the outbreak of pain,reduce the amount of analgesics,prolong sleep time,improve nursing satisfaction,and is safe and reliable.
