摘要
目的:通过研究2016-2020年口腔卫生资源配置现状分析口腔卫生资源配置的公平性及服务利用状况,为口腔卫生资源的合理分配提供科学依据,进一步促进口腔事业的发展。方法:运用Gini系数、泰尔指数、卫生资源密度指数和洛伦兹曲线对口腔卫生资源配置按照人口、地理和经济3个维度分别进行公平性分析,使用描述性统计分析法对服务利用状况展开分析。结果:Gini系数指出口腔卫生资源公平性为经济>人口>地理,其中经济和人口维度均在绝对公平状态(0.2),地理维度远超警戒线(0.6)。泰尔指数提示人口和经济维度不公平性主要来自区域内,地理维度不公性主要来自区域间。卫生资源密度增长迅速,年均增长率为18.15%,但服务利用效率偏低。结论:口腔卫生资源整体稳步增长,在资源配置中需要更加注重对农村地区和西部地区的倾斜,逐步引导口腔卫生资源向西部地区流动;行政区域上应合理规划口腔卫生资源分布,加强社会办医与口腔医院的联系,加强人才地域性流动。
Objective:To analyze the equity of oral health resource allocation and service utilization status by studying the current situation of oral health resource allocation in China from 2016 to 2020,so as to provide a scientific basis for the rational allocation of oral health resources and further promote the development of oral business in China.Methods:The Gini coefficient,Thayer index,health resource density index and Lorenz curve were used to analyze the equity of oral health resource allocation according to the three dimensions of population,geography and economy,and descriptive statistical analysis was used to analyze the service utilization status.Results:The Gini coefficient indicates that the fairness of oral health resources is that the economy is greater than the population,and the population is greater than the geography.The economic and population dimensions are both in an absolutely fair state(0.2),and the geographical dimension is far beyond the warning line(0.6).The Thiel index suggests that the inequity of the population and economic dimensions mainly comes from within regions,and the inequity of the geographic dimension mainly comes from between regions.Health resource density is growing rapidly with an average annual growth rate of 18.15%,but service utilization efficiency is low.Conclusion:Oral health resources as a whole are growing steadily,but more attention needs to be paid to rural areas and western regions in resource allocation,and gradually guide the flow of oral health resources to western regions;the distribution of oral health resources should be planned rationally in administrative regions,and the link between socially run hospitals and oral hospitals should be strengthened,and the geographical flow of talents should be enhanced.
