摘要
目的:分析脊柱感染性疾病MRI影像学征象及诊断意义.方法:选取2020年1月至2022年1月本院收治的疑似脊柱感染性疾病131例,后经临床证实有71例脊柱感染性疾病,均行MRI检查.分析MRI图像,计算MRI对脊柱感染性疾病检出率以及诊断效能.结果:MRI对脊柱结核、化脓性脊柱炎、真菌感染检出率分别为93.75%、84.61%、70.00%,与病理检查比较,差异无统计学意义(P>0.05);MRI对脊柱感染性疾病诊断灵敏度为77.46%、特异度为86.67%、阳性预测值为87.30%、阴性预测值为76.47%.71例患者中经MRI检查共累及椎体数为143个;64例患者病变椎体中可见信号不均匀,表现为T1、T2长信号为主,在T2WI中表现为片状低信号;增强扫描后病灶表现为轻-中度不均匀强化.结论:MRI检查可了解脊柱感染性疾病椎体以及周围组织的受累情况,对脊柱感染性疾病诊断效能高.
Objective:To analyze the efficacy and imaging characteristics of magnetic resonance imaging(MRI)in the diagnosis of spinal infectious diseases.Methods:One hundred and thirty-one patients with suspected spinal infectious diseases who were treated in our hospital from January 2020 to January 2022 were selected,and 71 cases of spinal infectious diseases were clinically confirmed.All of them underwent MRI examination.MRI images were analyzed,and detection rate and diagnostic efficiency of MRI for spinal infectious diseases were calculated.Results:The detection rates of MRI for spinal tuberculosis,suppurative spondylitis and fungal infection were 93.75%,84.61%,and 70.00%,respectively,and there were no differences with pathological examination(P>0.05);the diagnostic sensitivity of MRI spinal infectious diseases was 77.46%,the specificity was 86.67%,the positive predictive value was 87.30%,and the negative predictive value was 76.47%.Among the 71 patients,143 vertebral bodies were involved by MRI examination;64 patients showed uneven signal in the lesioned vertebral bodies,mainly manifested as long signals on T1 and T2,and patchy low signals on T2Wl;the lesions showed mild-moderate inhomogeneous enhancement after enhanced scanning.Conclusion:The involvement of the vertebral body and surrounding tissues of spinal infectious diseases,which has high diagnostic efficiency for spinal infectious diseases can befigured out with MRI.
