微视频结合行动导向教学对首次血液透析患者医学行为及自我效能的影响

摘要 目的 探究微视频结合行动导向教学对首次血液透析患者医学行为及自我效能的影响。方法 选择2018年1月至2021年4月在南通市第三人民医院血液净化中心进行首次血液透析的287例患者为研究对象,根据健康宣教的方式分为联合组157例与导向组130例,导向组施行行为导向的教学方式,联合组施行微视频结合行为导向的教学方式,对比两组患者干预前后自我管理能力(自我管理量表)、生活质量[肾脏疾病特异性调查表(KDQ)]、自我效能感[自我效能感量表(CPSS)]及满意度。结果 两组患者干预后合理饮食、水分及体质量控制、遵医服药、充足睡眠、正确对待疾病及自我监测能力等维度评分均上升,且干预后联合组患者上述自我管理能力维度评分均高于导向组(P<0.05);两组患者干预后躯体症状、疲劳、与他人关系及挫折等维度评分均上升,且干预后联合组患者上述生活质量维度评分均高于导向组(P<0.05);两组患者干预后疼痛应对、躯体功能及症状应对等维度评分均上升,且干预后联合组患者上述自我效能感维度评分均高于导向组(P<0.05);干预后联合组患者满意度高于导向组(P<0.05)。结论 微视频结合行动导向教学可提高首次血液透析患者自我管理能力与自我效能感,提高其生活质量,患者满意度更高。 Objective To explore the effect of micro-video combined with action-oriented teaching on medical behaviors and self-efficacy of patients with first-time hemodialysis. Methods Two hundred and eighty-seven patients who underwent hemodialysis for the first time in the hospital between January 2018 and April 2021 were selected as the research subjects. According to the health education methods, the patients were divided into combined group(157 cases) and oriented group(130 cases). The oriented group performed action-oriented teaching method, and the combined group implemented micro-video combined with action-oriented teaching method. The self-management ability(self-management scale), quality of life(kidney disease questionnaire [KDQ]), self-efficacy(chronic pain self-efficacy scale [CPSS]) and satisfaction before and after intervention were compared between the two groups. Results The scores of dimensions of reasonable diet, water content and weight control, medication compliance, adequate sleep, correct disease treatment and self-monitoring ability after intervention were increased in the two groups of patients, and after intervention, the scores of the above-mentioned dimensions of self-management ability of combined group were higher than those of oriented group(P<0.05). The scores of dimensions of physical symptoms, fatigue, relationship with others and frustration were all increased after intervention of the two groups, and the scores of the above-mentioned quality of life dimensions of patients in combined group were higher than those in oriented group(P<0.05). The scores of pain coping, physical function and symptom coping were increased in the two groups after intervention, and the scores of above-mentioned self-efficacy of combined group after intervention were all higher than those of oriented group(P<0.05). After intervention, the satisfaction of combined group was higher than that of oriented group(P<0.05).
