577 nm微脉冲激光治疗糖尿病重度黄斑水肿的效果及对黄斑厚度的影响 被引量:2

摘要 目的探讨577 nm微脉冲激光治疗糖尿病重度黄斑水肿的效果及对黄斑厚度的影响。方法选取2016年4月至2021年5月我院收治的100例糖尿病重度黄斑水肿患者为研究对象,根据入院编号的奇偶性将其分为对照组和研究组,各50例。对照组行常规抗血管内皮生长因子(VEGF)治疗,研究组在对照组基础上联用577 nm微脉冲激光治疗。比较两组的治疗效果。结果研究组的治疗总有效率高于对照组(P<0.05)。治疗后1、3、6个月,研究组的视力优于对照组(P<0.05)。治疗后,研究组的色素上皮衍生因子(PEDF)水平高于对照组,VEGF、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平低于对照组(P<0.05)。治疗后1、3、6个月,研究组的黄斑厚度及黄斑体积低于对照组(P<0.05)。结论577 nm微脉冲激光+抗VEGF治疗糖尿病重度黄斑水肿的效果显著,可降低黄斑厚度,改善视力,值得临床推广应用。 Objective To investigate the effect of 577 nm micropulse laser in the treatment of diabetic severe macular edema and its influence on macular thickness.Methods A total of 100 patients with diabetic severe macular edema admitted in our hospital from April 2016 to May 2021 were selected as the research objects.According to the parity of admission number,the patients were divided into control group and study group,with 50 cases in each group.The control group was treated with conventional anti-vascular endothelial growth factor(VEGF),and the study group was treated with 577 nm micropulse laser on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the study group was better than that in the control group(P<0.05).At 1,3 and 6 months after treatment,the visual acuity of the study group was higher than that of the control group(P<0.05).After treatment,the level of pigment epithelium-derived factor(PEDF)in the study group was higher than that in the control group,and the levels of VEGF,interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)were lower than those in the control group(P<0.05).At 1,3 and 6 months after treatment,the macular thickness and macular volume of the study group were lower than those of the control group(P<0.05).Conclusion 577 nm micropulse laser+anti-VEGF has a significant effect in the treatment of diabetic severe macular edema,which can reduce the macular thickness,improve the visual acuity.It is worthy of clinical promotion and application.
