基于三种微小RNA预测感染性心内膜炎并发急性肾损伤的临床价值 被引量:1

Clinical value of three microRNAs in predicting acute kidney injury in patients with infective endocarditis

摘要 目的探讨三种微小RNA预测感染性心内膜炎患者并发急性肾损伤(AKI)的临床价值。方法选取2017年1月-2020年10月四川省人民医院收治的111例感染性心内膜炎患者,根据患者是否并发AKI分为AKI组38例和非AKI组73例。采用实时荧光定量聚合酶链式反应法检测miR-214-5p、miR-192-5p、miR-200a-3p表达。比较两组患者一般资料、三种微小RNA表达水平以及肾功能指标。结果AKI组患者中的营养不良、贫血、机械通气时间≥24 h、二次气管插管的比例高于非AKI组患者(P<0.05)。AKI组的血清Scr、BUN、CyC、α1-MG水平以及尿微量白蛋白水平高于非AKI组(P<0.05)。AKI组的miR-214-5p、miR-192-5p表达水平高于非AKI组,miR-200a-3p表达水平低于非AKI组(P<0.05)。血清Scr、BUN、CyC、α1-MG以及尿微量白蛋白水平与miR-214-5p、miR-192-5p表达水平呈正相关,与miR-200a-3p表达水平呈负相关(P<0.05)。机械通气时间≥24 h、二次气管插管是感染性心内膜炎并发AKI的影响因素(P<0.05)。miR-214-5p、miR-192-5p、miR-200a-3p预测感染性心内膜炎并发AKI的敏感度为73.68%、63.16%、78.95%,特异度为69.86%、73.97%、54.79%。结论感染性心内膜炎并发AKI患者的miR-214-5p、miR-192-5p表达上调,miR-200a-3p表达下调,三种微小RNA对预测感染性心内膜炎并发AKI有较好的临床价值。 OBJECTIVE To investigate the clinical value of three microRNAs in predicting acute kidney injury(AKI)in patients with infective endocarditis.METHODS A total of 111 patients with infective endocarditis between Jan 2017 and Oct 2020 in Sichuan Provincial People’s Hospital were enrolled in this study.According to the presence or absence of AKI,the patients were divided into the AKI group(38 cases)and non-AKI group(73 cases).The expression of miR-214-5 p,miR-192-5 p and miR-200 a-3 p was detected by real-time fluorescent quantitative PCR.The general data,expression levels of the three microRNAs and renal function indicators were compared between the two groups.RESULTS The proportions of patients with malnutrition,anemia,mechanical ventilation time≥24 h,and secondary tracheal intubation in the AKI group were significantly higher than those in the non-AKI group(P<0.05).The levels of serum Scr,BUN,CyC,α1-MG and urine microalbumin in the AKI group were significantly higher than those in the non-AKI group(P<0.05).The expression levels of miR-214-5 p and miR-192-5 p in the AKI group were significantly higher than those in the non-AKI group,and the expression of miR-200 a-3 p was lower than that in the non-AKI group(P<0.05).The serum Scr,BUN,CyC,α1-MG and urine microalbumin levels were positively correlated with the expression levels of miR-214-5 p and miR-192-5 p,while negatively correlated with the expression level of miR-200 a-3 p(P<0.05).Mechanical ventilation time≥24 h,secondary tracheal intubation were important influencing factors for infective endocarditis complicated with AKI(P<0.05).The sensitivity of miR-214-5 p,miR-192-5 p and miR-200 a-3 p levels in predicting infective endocarditis with AKI was 73.68%,63.16%,and 78.95%,respectively,with the the specificity of 69.86%,73.97%and 54.79%,respectively.CONCLUSION The expression of miR-214-5 p and miR-192-5 p is up-regulated,while the expression of miR-200 a-3 p is down-regulated in patients with infective endocarditis complicated by AKI.The three microRNAs ar
