摘要
目的:探讨左西孟旦对Ⅰ型心肾综合征(CRSⅠ)患者心、肾功能的影响。方法:选取2019年1月-2021年7月山东省威海市立医院收治的78例CRSⅠ患者,随机分为观察者和对照组,各39例。对照组给予多巴胺治疗,观察组给予左西孟旦治疗。比较两组治疗前及治疗后1 d、3 d、7 d左心室射血分数(LVEF)、血浆氨基末端B型利钠肽前体(NT-proBNP)、胱抑素C(CysC)、血肌酐(Scr)水平变化。结果:治疗前,两组患者LVEF、NT-proBNP、CysC和Scr水平比较,差异无统计学意义(P>0.05);治疗后1 d、3 d、7 d,观察组LVEF水平均高于对照组,NT-proBNP、CysC和Scr水平均低于对照组,差异有统计学意义(P<0.05)。结论:左西孟旦在改善CRSⅠ患者心功能的同时,可明显改善肾功能,防止病情恶化,最大限度地减少心血管并发症的发生,提高患者生存质量,且安全性好,值得推广。
Objective:To investigate the effect of levosimendan on the cardiac and renal functions of patients with cardiorenal syndrome type 1(CRS1).Methods:A total of 78 CRS1 patients admitted to Weihai Municipal Hospital from January 2019 to July 2021 were selected and they were randomly divided into observation group and control group,with 39 cases in each group.The control group was treated with dopamine and the observation group was treated with levosimendan.The changes of left ventricular ejection fraction(LVEF),plasma amino-terminal pro-B-type natriuretic peptide(NT-proBNP),cystatin C(Cys C),serum creatinine(Scr)levels before treatment and at 1 d,3 d,7 d after treatment in two groups were compared.Results:Before treatment,there was no significant difference in the levels of LVEF,NT-proBNP,Cys C and Scr between the two groups(P>0.05).At 1 d,3 d,and 7 d after treatment,the LVEF levels in the observation group were higher than those in the control group,and the levels of NT-proBNP,Cys C and Scr were lower than those in the control group,with statistically significant differences(P<0.05).Conclusion:Levosimendan can improve the cardiac function of CRSI patients,significantly improve the renal function,prevent the deterioration of the disease,minimize the occurrence of cardiovascular complications,improve the quality of life of patients,and have good safety,which is worth promoting.
