摘要
目的:观察同仁大活络丸联合穴位注射治疗膝骨关节炎的临床疗效。方法:将160例中医辨证为风寒湿痹证的膝骨关节炎患者随机分为治疗组和对照组,每组80例。对照组除基础治疗外,温水送服同仁大活络丸治疗,每次2丸,早、晚各1次。治疗组在对照组的基础上联合穴位注射治疗,取双侧足三里穴位,注射用鹿瓜多肽8 mg以质量分数为0.9%的氯化钠注射液4 mL溶解,每穴注射2 mL,每周3次。2组均以4周为1个疗程。观察2组临床疗效,以及治疗前后膝关节功能评分变化。结果:治疗组治愈0例,显效1例,有效58例,无效21例,总有效率为73.75%;对照组治愈0例,显效0例,有效53例,无效27例,总有效率为66.25%。2组比较,差异有统计学意义(P<0.05)。治疗后,2组视觉模拟评分法评分、骨关节炎严重指数评分、西安大略和麦克马斯特大学骨关节炎指数(WOMAC)评分较治疗前均显著降低(P<0.05),且治疗组低于对照组(P<0.05);WOMAC评分中关节疼痛、僵硬及日常活动评分较治疗前显著降低(P<0.05),且治疗组低于对照组(P<0.05)。结论:穴位注射联合同仁大活络丸治疗膝骨关节炎,可提高疗效,改善关节疼痛、僵硬及日常活动情况,具有简便性、安全性和有效性,值得临床推广应用。
Objective:To observe the effect of Tongren Dahuoluo Wan(同仁大活络丸)combined with acupoint injection or Tongren Dahuoluo Wan alone on knee osteoarthritis.Methods:One hundred and sixty cases identified as knee osteoarthritis were randomly divided into a treatment group and a control group,40 cases in each.In addition to basic treatment,the control group was treated with Tongren Dahuoluo Wan,two pills each time,once for morning and evening respectively.The treatment group was treated with acupoint injection on the basis of the treatment for the control group.The bilateral Zusanli(ST 36)were taken to inject 8mg of Cervus and Cucumis Polypeptide for Injection which was dissolved with 4 mL of 0.9%sodium chloride injection.Each acupoint was injected with 2 mL,three times a week.Two groups were treated for four weeks.The clinical efficacy of both groups and the changes of knee function scores before and after treatment were observed.Results:In the treatment group,no cases were cured,1 case was markedly effective,58 cases were effective,21 cases were ineffective,and the total effective rate was 73.75%;in the control group,no cases were cured,no cases were markedly effective,53 cases were effective,27 cases were ineffective,and the total effective rate was 66.25%.The difference between the two groups was statistically significant(P<0.05).After treatment,VAS score,ISOA score and WOMAC score in both groups were significantly lower than those before treatment(P<0.05),and the treatment group was lower than the control group(P<0.05);The score of joint pain,stiffness and daily activities in the WOMAC osteoarthritis index score was significantly lower than that before treatment(P<0.05),and the treatment group was lower than the control group(P<0.05).Conclusion:Acupoint injection combined with Tongren Dahuoluo Wan can improve the curative effect of knee osteoarthritis,and it is expected to improve joint pain,stiffness and daily activities of patients through conservative treatment.Tongren Dahuoluo Wan combined with acupoin
