维生素D联合硫酸镁对重症毛细支气管炎患儿JAK/STAT信号通路与25(OH)D水平的影响 被引量:4

摘要 目的探讨维生素D联合硫酸镁对重症毛细支气管炎患儿Janus激酶/信号转导和转录激活因子(JAK/STAT)信号通路与25-羟维生素D_(3)[25-(OH)D_(3)]水平的影响。方法选取2021年2月至2022年2月沧州市人民医院收治的重症毛细支气管炎患儿93例作为研究对象,开展前瞻性研究,采用随机数字表法分为实验组(n=46)和对照组(n=47)。对照组采取硫酸镁治疗,实验组采取维生素D联合硫酸镁治疗。治疗2周后,观察并对比两组患儿临床疗效、临床症状消失时间,治疗前后血氧分压、经皮血氧饱和度、呼吸窘迫评分、JAK/STAT信号通路与25-(OH)D_(3)水平变化。结果实验组患儿治疗总有效率为95.65%明显高于对照组(78.72%),差异有统计学意义(P<0.05)。实验组患儿肺部炎症吸收时间、肺部湿啰音消失时间、咳嗽消失时间均早于对照组,差异均有统计学意义(P<0.05)。治疗2周后,两组患儿动脉血氧分析及经皮血氧饱和度明显高于治疗前,呼吸窘迫评分体系评分低于治疗前,且实验组患儿动脉血氧分析及经皮血氧饱和度为(8.25±1.24)kPa、(97.51±4.86)%,明显高于对照组[(7.14±1.15)kPa、(88.24±5.79)%],呼吸窘迫评分体系评分为(3.29±0.67)分,低于对照组[(5.37±0.85)分],差异均有统计学意义(P<0.05)。治疗2周后,两组患儿JAK2、STAT1、STAT2表达均高于治疗前,且实验组患儿JAK2、STAT1、STAT2表达为0.59±0.11、0.51±0.15、0.62±0.16,均高于对照组(0.42±0.10、0.38±0.12、0.43±0.17),差异均有统计学意义(P<0.05)。治疗2周后,两组患儿免疫球蛋白(Ig)A、25-(OH)D_(3)、IgG水平明显高于治疗前,且实验组患儿IgA、25-(OH)D_(3)、IgG水平为(2.21±0.27)g/L、(62.59±7.69)ng/L、(9.89±1.47)g/L,明显高于对照组[(1.79±0.32)g/L、(45.42±7.21)ng/L、(8.52±1.86)g/L],差异均有统计学意义(P<0.05)。两组不良反应情况相比,差异无统计学意义(P>0.05)。结论维生素D联合硫酸镁用于重症毛细支气管炎患� Objective To investigate the effect of vitamin D combined with magnesium sulfate on Janus kinase/activator of signal transduction and transcription(JAK/STAT)signal pathway and 25-hydroxyvitamin D_(3)[25-(OH)D_(3)]level in children with severe bronchiolitis.Methods A total of 93 children with severe bronchiolitis treated in Cangzhou People's Hospital from February 2021 to February 2022 were selected as the research objects to carry out a prospective study.They were randomly divided into experimental group(n=46)and control group(n=47).The control group was treated with magnesium sulfate,and the experimental group was treated with vitamin D combined with magnesium sulfate.After 2 weeks of treatment,the clinical efficacy,the time of disappearance of clinical symptoms,the changes of blood oxygen partial pressure,percutaneous oxygen saturation,respiratory distress score,jak/stat signal pathway and 25(OH)D_(3)level were observed and compared between the two groups.Results The total effective rate of children in the experimental group was 95.65%,which was significantly higher than in the control group(78.72%),the difference was statistically significant(P<0.05).The absorption time of lung inflammation,the disappearance time of lung moist rales and the disappearance time of cough in the experimental group were earlier than those in the control group,and the differences were statistically significant(P<0.05).After 2 weeks of treatment,the arterial blood oxygen analysis and transcutaneous oxygen saturation of the children in the two groups were significantly higher than those before treatment,and the respiratory distress scoring system score was lower than that before treatment,and the arterial blood oxygen analysis and transcutaneous oxygen saturation of the children in the experimental group were(8.25±1.24)kPa,(97.51±4.86)%,which were significantly higher than those of the control group[(7.14±1.15)kPa,(88.24±5.79)%],and the respiratory distress scoring system score was(3.29±0.67)points,which were lower than that of t
