氨溴索联合仙璐贝治疗慢性鼻-鼻窦炎患儿的效果及对炎性因子、免疫功能的影响 被引量:3

Effect of Ambroxol Combined with Xianlubei in the Treatment of Children with Chronic Rhinosinusitis and Its Effects on Inflammatory Factors and Immune Function

摘要 目的探讨氨溴索联合仙璐贝治疗慢性鼻-鼻窦炎患儿的临床效果及对Toll样受体9(TLR-9)、高迁移率族蛋白1(HMGB1)、转化生长因子-β1(TGF-β1)及炎性因子水平的影响。方法选取2018年5月—2020年5月收治慢性鼻-鼻窦炎患儿100例,按照治疗方法分为对照组和观察组各50例,对照组给予氨溴索治疗,观察组在对照组基础上给予仙璐贝治疗。比较2组治疗后外周血参数变化情况、视觉模拟评分法(VAS)评分、鼻内镜检查量化系统(Lund-Lennedy)评分、鼻窦CT评分、TLR-9、白细胞介素-17(IL-17)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、鼻纤毛传输功能、HMGB1、TGF-β1水平,观察2组治疗效果及不良反应发生情况。结果治疗后观察组白细胞计数、中性粒细胞计数、中性粒细胞计数/淋巴细胞计数低于对照组,淋巴细胞计数高于对照组(P<0.01)。治疗后观察组VAS评分、Lund-Lennedy评分、鼻窦CT评分低于对照组(P<0.01)。观察组治疗后IL-17、CRP、TNF-α、HMGB1水平及鼻纤毛传输功能低于对照组,TLR-9、TGF-β1高于对照组(P<0.05,P<0.01)。观察组总有效率高于对照组(P<0.05)。2组不良反应发生率比较差异无统计学意义(P>0.05)。结论氨溴索联合仙璐贝可有效改善慢性鼻-鼻窦炎患儿的临床症状、鼻黏膜纤毛传输功能,增强机体免疫功能,减轻机体炎症反应。 Objective To investigate the clinical effect of Ambroxol combined with Xianlubei in the treatment of children with chronic rhinosinusitis(CRS)and its effects on Toll-like receptor 9(TLR-9),high mobility group box 1(HMGB1),transforming growth factor-β1(TGF-β1)and the level of inflammatory factors.Methods We selected 100 children with CRS who were treated from May 2018 to May 2020,and divided them into the control group(n=50)and the observation group(n=50)according to the treatment method.The control group was given Ambroxol,and the observation group was given Xianlubei treatment on the basis of the control group.The changes of peripheral blood parameters,visual analogue scale(VAS)score,nasal endoscopy quantification system(Lund-Lennedy)score,sinus CT score,TLR-9,interleukin-17(IL-17)score,C reactive protein(CRP),tumor necrosis factor-α(TNF-α),nasal mucociliary transport function,HMGB1,TGF-β1 levels were compared between the two groups after treatment,and the therapeutic effects and adverse reactions of the two groups were observed.Results After treatment,the white blood cell(WBC)count,neutrophil count and neutrophil to lymphocyte ratio(NLR)in the observation group were lower than those in the control group,and the lymphocyte count was higher than that in the control group(P<0.01).After treatment,the VAS score,Lund-Lennedy score and sinus CT score of the observation group were lower than those of the control group(P<0.01).After treatment,the levels of IL-17,CRP,TNF-α,HMGB1 and the nasal mucociliary transport function in the observation group were lower than those in the control group,while TLR-9 and TGF-β1 were higher than those in the control group(P<0.05,P<0.01).The total effective rate in the observation group was higher than that in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Ambroxol combined with Xianlubei can effectively improve the clinical symptoms and nasal mucociliary transport function of children wi
