摘要
目的研究三维斑点追踪技术联合Tei指数在急性心肌梗死(Acute Myocardial Infarction,AMI)患者经皮冠状动脉介入(Percutaneous Transluminal Coronary Intervention,PCI)术后心功能评估中的应用价值。方法选取2019年1月至2021年12月河北北方学院附属第一医院收治的90例经PCI治疗的AMI患者为研究对象。所有患者均行超声检查,检测左心室收缩末期内径(Left Ventricular End Systolic Diameter,LVESD)、左心室舒张末期内径(Left Ventricular End Diastolic Diameter,LVEDD)、左室射血分数(Left Ventricular Ejection Fraction,LVEF)、左心室收缩末容积(Left Ventricular End Systolic Volume,LVESV)及左心室舒张末容积(Left Ventricular End Diastolic Volume,LVEDV),并计算Tei指数。采取实时三维斑点追踪成像获取左心室收缩期整体纵向应变(Global Longitudinal Strain,GLS)、整体径向应变(Global Radial Strain,GRS)、整体面积应变(Global Area Strain,GAS)、整体圆周应变(Global Circumferential Strain,GCS)。比较患者PCI术后GLS、GRS、GCS、GAS及Tei指数变化,并观察患者心功能,通过相关性分析明确三维斑点追踪成像指标及Tei指数与患者心功能的关系。结果患者术后LVEDD、LVEDV、LVESV水平及GLS、GRS、GCS、GAS、Tei指数均有显著变化,经相关性分析证实GLS、GCS、GAS及Tei指数均与LVEDD、LVESD、LVEDV、LVESV呈正相关(P<0.05),与LVEF呈负相关(P<0.05);GRS与LVEDD、LVESD、LVEDV、LVESV呈负相关(P<0.05),与LVEF呈正相关(P<0.05)。结论三维斑点追踪技术可用于评估AMI患者PCI术后心功能,结合Tei指数可进一步提高评估效能,帮助早期发现患者心功能改变。
Objective To study the value of three-dimensional speckle tracking combined with Tei index in the evaluation of cardiac function after percutaneous coronary intervention(PCI)in patients with acute myocardial infarction(AMI).Methods A total of 90 AMI patients treated with PCI in the First Affiliated Hospital of Hebei North University from January 2019 to December 2021 were selected as the research subjects.All patients were examined by echocardiography.Left ventricular end diastolic diameter(LVEDD),left ventricular end systolic diameter(LVESD),left ventricular end diastolic volume(LVEDV),left ventricular end systolic volume(LVESV)and left ventricular ejection fraction(LVEF)were measured,and Tei index was calculated.Left ventricular systolic global longitudinal strain(GLS),global radial strain(GRS),global circumferential strain(GCS)and global area strain(GAS)were obtained by real-time three-dimensional speckle tracking imaging.The changes of GLS,GRS,GCS,GAS and Tei index after PCI were compared,and the cardiac function was observed.The relationship between three-dimensional speckle tracking imaging index,Tei index and cardiac function was determined through correlation analysis.Results The levels of LVEDD,LVEDV,LVESV and GLS,GRS,GCS,GAS and Tei indexes were significantly changed after operation.The correlation analysis showed that GLS,GCS,GAS and Tei indexes were positively correlated with LVEDD,LVESD,LVEDV and LVESV(P<0.05),and negatively correlated with LVEF(P<0.05);GRS was negatively correlated with LVEDD,LVESD,LVEDV,LVESV(P<0.05),and positively correlated with LVEF(P<0.05).Conclusion Three-dimensional speckle tracking technology can be used to evaluate the cardiac function of AMI patients after PCI.Combined with Tei index,the evaluation efficiency can be further improved to help early detection of cardiac function changes in patients.
