不同基因亚型人乳头瘤病毒感染与宫颈病变的关系研究

摘要 目的 分析不同基因亚型人乳头瘤病毒感染(HPV)与宫颈病变的关系。方法 以符合标准的100例宫颈病变者进行分析,病例选择时间2017年1月至2021年5月,对患者进行液基细胞学检查及HPV检测,分析不同基因亚型HPV感染与宫颈病变间的关系。结果 100例宫颈病变者,HPV感染59例,阳性率为59.0%。宫颈炎HPV感染阳性率28.89%,CINⅠHPV感染阳性率为70.0%,CINⅡHPV感染阳性率为86.67%,CINⅢHPV感染阳性率为91.67%,宫颈癌HPV感染阳性率为100.0%。病情加重,阳性率递增(P<0.05);宫颈病变中常见基因亚型为HPV16、18、33、52、58,其中HPV16感染的检出率最高,宫颈炎为2.22%、CINⅠ为5.0%,CINⅡ为33.33%,CINⅢ为51.42%,宫颈癌为85.71%,随病变程度加重,检出率明显增高,差异有统计学意义(P<0.05);不同宫颈病变患者,其单一感染与多重感染,差异无统计意义(P>0.05)。结论 HPV阳性率可随患者宫颈病变病情的加重逐渐增高,HPV16、18、33、52、58为宫颈病变患者多见HPV基因亚型,其中HPV16在病程演变中起到重要作用,属于高危型HPV,与病变程度正相关。 Objective To analyze the relationship between different genotypes of human papillomavirus(HPV) infection and cervical lesions. Methods 100 cases of cervical lesions meeting the standard were analyzed. The case selection time was from January 2017 to may 2021. The patients were examined by liquid-based cytology and HPV detection to analyze the relationship between HPV infection of different gene subtypes and cervical lesions.Results Among 100 cases of cervical lesions, 59 cases were infected with HPV, and the positive rate was 59.0%. The positive rate of HPV infection was 28.89% in cervicitis, 70.0% in CIN Ⅰ HPV infection, 86.67% in CIN Ⅱ HPV infection, 91.67% in CIN Ⅲ HPV infection, and 100.0% in cervical cancer. The disease worsened, and the positive rate increased(P < 0.05). The common genotypes in cervical lesions were HPV16, 18, 33, 52, and 58.The detection rate of HPV16 infection was the highest, cervicitis was 2.22%, and CIN Ⅰ was 5.0%. CIN II was 33.33%, CIN Ⅲ was 51.42%, and cervical cancer was 85.71%. The detection rate increased significantly with the severity of the lesions(P < 0.05). There was no significant difference between single infection and multiple infection in patients with different cervical lesions.(P > 0.05).Conclusion The positive rate of HPV can gradually increase with the aggravation of cervical lesions. HPV16, 18, 33, 52 and 58 are common HPV gene subtypes in patients with cervical lesions. HPV16 plays an important role in the evolution of the course of disease and belongs to high-risk HPV, which is positively correlated with the degree of lesions.
