早期肠内营养支持治疗重症脑出血患者的临床效果 被引量:4

摘要 目的:分析早期肠内营养支持治疗重症脑出血患者的临床效果。方法:选取2018年11月-2021年10月北京市泰康燕园康复医院收治的重症脑出血患者70例为观察对象,根据随机抽签法分为对照组及观察组,各35例。对照组采取早期肠外营养支持,观察组采取早期肠内营养支持。比较两组患者身体营养指标、神经功能及生活自理能力、并发症发生率及住院指标。结果:干预后,两组患者体重指数(BMI)、血红蛋白(Hb)及血清白蛋白(ALB)水平均降低,观察组BMI、Hb、ALB水平高于对照组,差异有统计学意义(P<0.05);干预后,两组患者美国国立卫生研究所制定的卒中量表(NIHSS)评分均降低,Barthel指数评分均升高,同时,观察组NIHSS评分低于对照组,Barthel指数评分高于对照组,差异均有统计学意义(P<0.05);观察组并发症发生率低于对照组,差异有统计学意义(P<0.05);观察组重症监护室(ICU)停留时间短于对照组,住院费用、病死率低于对照组,死亡病例存活时间高于对照组,差异有统计学意义(P<0.05)。结论:对重症脑出血患者采取早期肠内营养支持,可改善身体营养状况,缩短治疗时间,改善患者神经功能,提升恢复效果及患者生活自理能力,降低临床病死率及并发症发生率。 Objective: To analyze the clinical effect of early enteral nutrition support in treatment of patients with severeintracerebral hemorrhage. Methods: From November 2018 to October 2021, 70 patients with severe intracerebral hemorrhage treatedin Beijing Taikang Yanyuan Rehabilitation Hospital were selected as the observation subjects. They were divided into control groupand observation group by random lot method, with 35 cases in each group. The control group received early parenteral nutritionsupport and the observation group received early enteral nutrition support. The physical nutritional indexes, neurological functionand self-care ability, incidence of complications and hospitalization index of two groups were compared. Results: After theintervention, the levels of body mass index(BMI), hemoglobin(Hb) and serum albumin(ALB) in the two groups were reduced, andthe BMI, Hb and ALB levels in the observation group were higher than those in the control group, with statistically significantdifferences(P<0.05). After the intervention, the National Institutes of Health stroke scale(NIHSS) scores in the two groups werereduced, and the Barthel score was increased;the NIHSS score in the observation group was lower than that in the control group, andthe Barthel score was higher than that in the control group, with statistically significant differences(P<0.05). The incidence ofcomplication in the observation group was lower than that in the control group, and the difference was statistically significant(P<0.05).The intensive care unit(ICU) stay time in the observation group was shorter than that in the control group, the hospitalization cost andmortality were lower than those in the control group, and the survival time of dead cases was longer than that in the control group, withstatistically significant differences(P<0.05). Conclusion: Early enteral nutrition support for patients with severe intracerebralhemorrhage can improve the nutritional status of the body, shorten the treatment time, improve the neurological function,
