摘要
目的探讨结直肠癌患者造口周围潮湿相关性皮肤损伤(PMASD)的影响因素,为建立相关预警机制及干预策略提供依据。方法采用回顾性分析法,通过临床大数据(医渡云)平台,提取某三级甲等肿瘤专科医院2019年11月-2021年10月医疗信息系统中结直肠癌造口患者的临床数据,采用单因素和logistic回归分析确定患者PMASD的影响因素。结果结直肠癌患者PMASD发生率为57.6%(269/467),logistic回归分析结果显示:年龄(OR=1.054,P<0.000)、空腹血糖(FPG;OR=4.886,P<0.001)、放疗史(OR=3.409,P<0.001)、造口种类(OR=0.490,P=0.047)、造口开口高度(OR=13.947,P<0.001)、造口周围皮肤褶皱(OR=2.780,P=0.003)、排便性状(OR=0.435,P=0.008)、预防性使用防漏膏(OR=0.211,P<0.001)是结直肠癌患者PMASD发生的影响因素。结论结直肠癌患者PMASD发生率较高,应针对其高危因素建立精准预警机制及干预策略,以降低其发生率。
Objective To explore the influencing factors of peristomal moisture-associated skin damage(PMASD)in patients with colorectal cancer,so as to provide a basis for establishing relevant early warning mechanism and intervention strategies.Methods Retrospective analysis was used to extract the clinical data of colorectal cancer patients with colostomy from the medical information system of a tertiary Grade A cancer hospital from November 2019 to October 2021 through the clinical big data(Yidu Cloud)platform.Univariate and logistic regression analysis were used to determine the influencing factors of PMASD.Results The incidence of PMASD in colorectal cancer patients was 57.6%(269/467),and logistic regression analysis showed that age(OR=1.054,P<0.001),fasting blood-glucose(FPG;OR=4.886,P<0.001),radiotherapy history(OR=3.409,P<0.001),stoma type(OR=0.490,P=0.047),stoma opening height(OR=13.947,P<0.001),skin folds around the stoma(OR=2.780,P=0.003),defecation characteristics(OR=0.435,P=0.008),and preventive use of anti-leakage ointment(OR=0.211,P<0.001)were the influencing factors for the occurrence of PMASD in colorectal cancer patients.Conclusion The incidence of PMASD in colorectal cancer patients is high,and accurate early warning mechanism and intervention strategy should be established to reduce the incidence of PMASD according to its high risk factors.
