摘要
目的 探讨血清胱抑素-C(Cys-C)、β淀粉样蛋白1-42(Aβl-42)及胰岛素样生长因子-1(IGF-1)在老年缺血性脑卒中的表达及临床意义。方法 2017年10月至2021年10月我院收治的缺血性脑卒中患者102例作为研究组,同期纳入在本院体检的正常人83例作为对照组。比较研究组与对照组、有无认知功能障碍脑卒中患者及不同神经功能缺损程度患者血清Cys-C、Aβl-42、IGF-1水平;绘制ROC曲线分析上述因子诊断缺血性脑卒中后认知功能障碍的价值。结果 研究组Cys-C水平明显高于对照组,Aβl-42及IGF-1水平低于对照组(P<0.05);障碍组Cys-C水平明显高于无障碍组,Aβl-42及IGF-1水平低于无障碍组(P<0.05);不同神经功能缺损程度脑卒中患者血清Cys-C水平比较:轻度组<中度组<重度组,Aβl-42及IGF-1水平比较:轻度组>中度组>重度组(P<0.05);ROC曲线结果显示,Cys-C、Aβl-42及IGF-1三者联合检测缺血性脑卒中后认知功能障碍的曲线下面积(AUC)、敏感度、特异度分别为0.919、0.852、0.899,均明显高于各项单一检测(P<0.05)。结论 Cys-C、Aβl-42及IGF-1水平在缺血性脑卒中患者存在异常表达,可能成为诊断缺血性脑卒中后认知功能障碍、评估患者病情的参考指标。
Objective To investigate the expressions and clinical significance of serum cystatin-C(Cys-C),β-amyloid 1-42(Aβl-42)and insulin like growth factor-1(IGF-1)in senior ischemic stroke.Methods One hundred and two ischemic stroke patients admitted to our hospital from October 2017 to October 2021 were selected as a study group.During the same period,83 normal people who underwent physical examination in our hospital were included as a control group.Serum levels of Cys-C,Aβl-42 and IGF-1 between the two groups,between stroke patients with and without cognitive dysfunction,and among patients with different degrees of neurological deficit were compared.ROC curves were drawn to analyze the value of the above factors in the diagnosis of cognitive dysfunction after ischemic stroke.Results The level of Cys-C was significantly higher and the levels of Aβl-42 and IGF-1 were lower in the study group than those in the control group(P<0.05).The level of Cys-C was significantly higher,and the levels of Aβl-42 and IGF-1 were lower in the cognitive dysfunction group than those in the non-cognitive dysfunction group(P<0.05).The serum Cys-C levels in patients with different degrees of neurological deficit in ischemic stroke was that the mild grou P<the moderate grou P<the severe group.The serum levels of Aβl-42 and IGF-1 were that the mild group>the moderate group>the severe group(P<0.05).The results of ROC curve analysis showed that the area under the ROC curve(AUC),sensitivity and specificity of combined detection of Cys-C,Aβl-42 and IGF-1 in the diagnosis of cognitive dysfunction after ischemic stroke were 0.919,0.852,and 0.899,respectively,which were significantly higher those that of single test.Conclusions The levels of Cys-C,Aβl-42 and IGF-1 are abnormally expressed in patients with ischemic stroke.The factors may become reference indicators for diagnosing cognitive dysfunction and evaluating patients′condition after ischemic stroke.
