摘要
目的寻找一种可以减轻逆行腓肠神经营养血管皮瓣传统手术方式后的蒂部肿胀畸形的方法,以求改善足踝部的功能和外观。方法在传统手术方式的基础上,对皮瓣的蒂部进行改良,改良方式为切断蒂部近端上旋转轴点的皮肤,从畸形部分的近处到远处、浅处到深处进行分离,由浅入深分离至到深层皮下筋膜组织时停止,由近及远分离至旋转轴点时停止,使近端蒂部”猫耳”样畸形的皮肤变成真皮层皮片。结果该例患者的皮瓣完全成活,创面完全愈合。皮瓣不会有“猫耳”样的畸形,也不会有明显的肿胀,功能和外观均较好。结论该蒂部改良方式能有效地减轻术后皮瓣的肿胀畸形,使足踝部恢复正常的功能,并改善足踝部的外观。
Objective To find one method to reduce the swelling and deformity of the pedicle of the reverse sural neurovascular flap after traditional operation,in order to improve the function and appearance of the ankle.Methods On the basis of the traditional surgical method,the pedicle of the flap was improved by cutting off the skin at the rotating axis point at the proximal end of the pedicle,separating the deformed part from near to far,from shallow to deep,from shallow to deep to deep subcutaneous fascia,and from near to far to rotating axis point,so that the"cat's ear"-like deformed skin at the proximal pedicle became dermis skin graft.Results The skin flap of this patient survived completely and the wound healed completely.The flap will not have"cat's ear"deformity,nor obvious swelling,and its function and appearance are good.Conclusion The improved pedicle method can effectively reduce the swelling and deformity of the skin flap after operation,restore the normal function of the foot and ankle,and improve the appearance of the foot and ankle,which has certain application value.
