摘要
目的研究补肾活血壮骨汤对骨质疏松性骨折术后骨折愈合及Ⅰ型胶原羧基端肽β特殊序列(β-CTX)及尿骨胶原交联(Crosslaps)的影响。方法选取陕西中医药大学附属医院于2018年1月至2019年2月收治的126例骨质疏松性桡骨骨折患者纳入研究,按照随机数表法将患者分为对照组和观察组,每组63例,对照组患者采用切开复位钢板螺钉内固定术治疗,观察组患者在对照组基础上于术后第一天开始服用补肾活血壮骨汤,每日1剂,4周一个疗程。连续服用3个疗程后,采用Gartland-Werler评分及PRWE评分评估并比较两组患者的腕关节功能情况,治疗前、后β-CTX与Crosslaps的表达水平,并比较两组术后并发症发生情况。结果观察组患者骨折愈合时间(12.35±1.01)周,明显短于对照组的(14.69±1.1)周,差异具有统计学意义(P<0.05);术后3个月,观察组患者的Gartland-Werler评分、PRWE评分分别为(7.98±1.22)分、(19.67±3.68)分,明显高于对照组的(6.89±1.34)分、(15.98±3.17)分,差异均有统计学意义(P<0.05);治疗前,两组患者的β-CTX、Crosslaps表达水平比较差异均无统计学意义(P>0.05);治疗后,两组患者的β-CTX、Crosslaps表达水平明显高于治疗前,且观察组明显高于对照组,差异均有统计学意义(P<0.05);观察组患者术后并发症总发生率为12.70%,明显低于对照组的28.57%,差异具有统计学意义(P<0.05)。结论骨质疏松性桡骨远端骨折术后使用补肾活血壮骨汤能够提高患者腕关节的恢复速度,并能够上调β-CTX、Crosslaps的表达,从而有效缩短患者骨性愈合的时间,快速恢复关节功能,减少术后并发症。
Objective To study the effects of Bushen Huoxue Zhuanggu Decoction on postoperative healing of osteoporotic fractures and serum beta C-terminal cross-linked telopeptides of typeⅠcollagen(β-CTX)and urine crosslaps(Crosslaps).Methods A total of 126 patients with osteoporotic radius fractures admitted to the Affiliated Hospital of Shaanxi University of Traditional Chinese Medicine from January 2018 to February 2019 were selected.According to the random number table method,the patients were divided into a control group and an observation group,with 63 patients in each group.Patients in the control group were treated with open reduction and steel plate screw internal fixation.Patients in the observation group were treated with Bushen Huoxue Zhuanggu Decoction(1 dose per day,4 weeks a course)on the first day after the operation on the basis of the control group.After 3 consecutive courses of treatment,Gartland-Werler score and paticnt rated wrist evaluation(PRWE)were used to evaluate and compare the wrist function between the two groups.The expression levels ofβ-CTX and Crosslaps were detected and recorded before and after treatment,and the incidence of postoperative complications were compared between the two groups.Results The fracture healing time in the observation group was(12.35±1.01)weeks,which was significantly shorter than(14.69±1.1)weeks in the control group(P<0.05).At 3 months after operation,the Gartland-Werler score and PRWE score in the observation group were(7.98±1.22)points and(19.67±3.68)points,respectively,which were significantly higher than(6.89±1.34)points and(15.98±3.17)points in the control group(P<0.05).Before treatment,there was no significant difference in the expression levels ofβ-CTX and Crosslaps between the two groups(P>0.05).After treatment,the expression levels ofβ-CTX and Crosslaps in the two groups were significantly higher than those before treatment,and the levels in the observation group were significantly higher than those in the control group,with statistically signifi
