摘要
便秘是恶性肿瘤患者常见的并发症和治疗相关不良反应。梳理总结中医药治疗恶性肿瘤相关性便秘的临床研究文献发现,其病因主要包括内因、外因。内因主要有饮食不节、情志失调、药物不良反应、劳倦久坐、先天禀赋不足、年老等;外因主要为外感六淫邪气,与寒、热、燥邪关系甚密。恶性肿瘤发病后机体发生器质性改变、抗癌治疗、相关并发症等易致便秘,加之癌毒耗伤,致大肠传导失司。病位在大肠,涉及肺、脾、胃、肝和肾,应以“通”立法。中医药治疗主要有中医内治法、外治法和内外治法结合的综合疗法,其中内治法包括经方和验方,外治法有穴位贴敷、灌肠、针灸等,均可明显缓解患者的便秘症状。中医药治疗恶性肿瘤相关性便秘疗效显著,但目前存在循证医学证据缺乏、宣传推广力度不足等问题和不足。参考文献51篇。
Constipation is a common complication and treatment-related adverse reaction in patients with malignant tumors.The author summarized the clinical research literature on traditional Chinese medicine(TCM)treatment of malignant tumor-related constipation.The results showed that the etiology mainly included internal and external causers.The main internal causes are improper diet,emotional disorders,adverse drug reactions,overstrain and lack of physical exercises,congenital endowment deficiency,old age,etc.The external causes mainly refer to six pathogenic qi,which are closely related to cold,heat and dryness.After the onset of malignant tumor,the body changes,anti-cancer treatment,and related complications can easily lead to constipation.In addition,cancer toxins can cause loss of conduction in the large intestine.The disease is located in the large intestine,involving the lung,spleen,stomach,liver and kidney and taking dredging as the treatment principle.TCM treatment mainly includes internal therapy,external therapy and comprehensive therapy.Among them internal therapy includes classic formulas and proven formulas,external therapy includes acupoint sticking,enema,acupuncture and moxibustion,etc.,which can significantly relieve the symptoms of constipation.TCM is effective in the treatment of malignant tumor-related constipation,but there are some problems such as lack of evidence of evidence-based medicine and insufficient publicity and promotion.There are 51 references in total.
