摘要
目的分析退行性腰椎管狭窄症(DLSS)减压术后患者发生下肢疼痛的相关因素。方法选取邓州市中心医院2018年12月至2021年6月收治的210例DLSS患者,患者接受腰椎管减压术治疗,且术后随访6个月,依据相关标准评估患者下肢疼痛发生情况并进行分组。设计患者基线资料填写表,询问并记录患者研究所需资料,将可能的影响因素纳入,分析可能导致DLSS患者减压术后下肢疼痛发生的相关因素。结果210例DLSS患者术后随访6个月,41例发生下肢疼痛,发生率为19.52%(41/210);发生组狭窄节段数量多于未发生组,术前有腰椎不稳、有脑脊液漏、瘢痕体质人数占比高于未发生组(P<0.05);组间其他基线资料比较,差异无统计学意义(P>0.05);经logistic回归分析,结果显示,术前腰椎不稳、狭窄节段数量多、有脑脊液漏、瘢痕体质是DLSS患者减压术后并发下肢疼痛的影响因素(OR>1,P<0.05)。结论DLSS患者术后发生下肢疼痛可能与术前腰椎不稳、狭窄节段数量多、有脑脊液漏以及瘢痕体质等因素有关。
Objective To analyze the related factors of lower limb pain in patients with degenerative lumbar spinal stenosis(DLSS)after decompression.Methods A total of 210 patients with DLSS treated in Dengzhou Central Hospital from December 2018 to June 2021 were selected.All patients were treated with lumbar spinal canal decompression in hospital,and were followed up for 6 months.The occurrence of lower limb pain of patients was evaluated and grouped according to relevant standards.The baseline data of patients was designed,and the required data of the patients were recorded.The possible influencing factors were included to analyze the influencing factors that might lead to the occurrence of lower limb pain in patients with DLSS after decompression.Results Among 210 patients with DLSS by assessment after 6 months of operation,41 had lower extremity pain,with an incidence of 19.52%(41/10).Compared with the non occurrence group,the number of stenosis segments in the occurrence group was larger.The proportion of patients with lumbar instability,cerebrospinal fluid leakage and scar constitution before operation was higher(P<0.05).There was no statistical difference in other baseline data between groups(P>0.05).The results of logistic regression analysis showed that,preoperative lumbar instability,a large number of narrow segments,cerebrospinal fluid leakage and scar constitution were the influencing factors of lower limb pain after decompression in patients with DLSS(OR>1,P<0.05).Conclusion The postoperative lower limb pain in patients with DLSS may be related to preoperative lumbar instability,a large number of narrow segments,cerebrospinal fluid leakage and scar constitution.
