摘要
目的 探讨凉血通瘀汤对脑出血(ICH)伴急性胃肠功能障碍患者的临床效果。方法 将62例ICH伴急性胃肠功能障碍患者采用随机数字表法分成观察组与对照组各31例。对照组予以西医常规治疗,观察组在对照组基础上采用凉血通瘀汤鼻饲治疗。比较两组治疗前后的急性胃肠功能损伤(AGI)分级、血清胃动素(MTL)和胃泌素(GAS)含量、胃肠屏障功能指标和临床指标评分。结果 观察组治疗后AGI改善程度明显优于对照组(P<0.05)。经过治疗后,两组MTL、GAS水平较治疗前均有降低(P<0.05),观察组降低程度更明显(P<0.05);两组胃肠屏障功能指标较治疗前明显下降(P<0.05),观察组下降程度更明显(P<0.05)。两组治疗后胃肠症状评分、NIHSS评分、中医证候评分较治疗前明显下降(P<0.05),观察组下降程度更明显(P<0.05)。结论 凉血通瘀汤有助于改善ICH伴胃肠功能障碍患者的胃肠功能损伤情况,降低血清MTL、GAS含量,有助于改善胃肠屏障功能、缓解病情严重程度。
Objective:To investigate the clinical effect of Liangxue Tongyu Decoction on patients with cerebral hemorrhage(ICH)and acute gastrointestinal dysfunction. Methods:62 patients with ICH complicated with acute gastrointestinal dysfunction were randomly divided into an observation group and a control group,with 31 cases in each group. The control group was given conventional western medicine treatment,and the observation group was given Liangxue Tongyu Decoction on the basis of nasal feeding. The acute gastrointestinal injury(AGI)grade,serum MTL and GAS levels,gastrointestinal barrier function indexes and clinical index scores were compared between the two groups before and after treatment. Results:After treatment,the improvement of AGI in the observation group was significantly better than that in the control group(P< 0.05). After treatment,the levels of MTL and GAS in both groups were decreased compared with those before treatment(P< 0.05),and the degree of decrease in the observation group was more obvious(P< 0.05). After treatment,the gastrointestinal barrier function indexes in the two groups were significantly decreased compared with those before treatment(P< 0.05),and the degree of decrease in the observation group was more obvious(P< 0.05). Gastrointestinal symptom scores,NIHSS scores,and TCM syndrome scores in the two groups after treatment were significantly decreased compared with those before treatment(P< 0.05),and the degree of decrease in the observation group was more obvious(P< 0.05). Conclusion:Liangxue Tongyu Decoction can help to improve the gastrointestinal function damage in patients with ICH complicated with gastrointestinal dysfunction,reduce serum MTL and GAS levels,help to improve gastrointestinal barrier function and relieve the severity of the disease.
