摘要
目的研究单核细胞与高密度脂蛋白胆固醇比值(MHR)对老年急性ST段抬高型心肌梗死(STEMI)患者经皮冠状动脉介入治疗(PCI)术后心血管不良事件(MACEs)的影响。方法选取2019年1月~2020年10月北京天坛医院438例接受急诊PCI的老年STEMI患者,根据入院时的MHR将患者分为低、中和高MHR 3组。比较和分析3组患者术后12个月内MACEs的发生情况。结果在12个月的随访中,低、中和高MHR3组中发生MACEs的患者分别有16例(11.1%)、29例(19.7%)和51例(34.7%),随着MHR的升高MACEs的发生率呈上升趋势,3组间MACEs发生率差异有统计学意义(P<0.05)。多因素Cox风险回归分析显示,MHR水平是老年STEMI患者PCI术后12个月内发生MACEs的独立预测因子(HR=5.49,95%CI:3.42~8.82,P<0.05)。结论在老年STEMI患者中,MHR是PCI术后MACEs发生的独立危险因素。
Objective The aim of this study is to investigate the effect of monocyte to high-density lipoprotein cholesterol ratio(MHR)on cardiovascular adverse events(MACEs)in elderly patients with acute ST segment elevation myocardial infarction(STEMI)undergoing percutaneous coronary intervention(PCI).Methods 438 elderly patients with STEMI who received successful primary PCI were enrolled from January 2019 to October 2020 in Beijing Tiantan hospital.Patients were divided into three groups,according to the MHR levels at admission.Patients were followed up for 12 months for MACEs.Results During the 12-moths follow-up,MACEs occurred in16 cases(11.1%)in group 1,29 cases(19.7%)in group 2 and 51 cases(34.7%)in group 3.There was significant difference in the occurrence of MACEs among the three groups(P<0.05).The occurrence of MACEs showed an upward trend,as the MHR increasing.Multivariate Cox regression analysis showed that MHR level was an independent predictor of MACEs in elderly STEMI patients(HR=5.49,95%CI:3.42-8.82,P<0.05).Conclusion MHR levels are strongly associated with poor prognosis in elderly STEMI patients undergoing PCI.
