动态对比增强MRI在肺良恶性结节(≤2cm)的诊断价值 被引量:2

Diagnostic Value of Dynamic Contrast-Enhanced MRI in Benign and Malignant Lung Nodules(Not Longer Than 2cm)

摘要 目的探究动态对比增强MRI(DCE-MRI)在≤2cm肺良恶性结节中的诊断价值。方法以2018年3月至2021年2月≤2cm孤立性肺结节患者101例为研究对象,均给予DCE-MRI检查及穿刺活检。根据穿刺活检结果确定结节良恶性及具体类型,比较良性结节及恶性结节的DCE-MRI参数[容积转运常数(K^(trans))、血液回流常数(K_(ep))、细胞外间隙对比剂容积分数(V_(e))、血浆内对比剂容积分数(Vp)],分析其对良恶性结节的鉴别价值;比较肺腺癌及肺鳞癌的DCE-MRI参数,分析其对肺鳞癌及肺腺癌的鉴别价值,并比较不同类型良性结节的DCE-MRI参数。结果恶性结节的K^(trans)、K_(ep)、V_(e)、V_(p)值大于良性结节(P﹤0.05);K^(trans)、K_(ep)、V_(e)、V_(p)值鉴别良恶性肺结节的AUC分别为0.774、0.781、0.719、0.670;肺鳞癌的K^(trans)、K_(ep)值小于肺腺癌(P﹤0.05);K^(trans)、K_(ep)、V_(e)、V_(p)值鉴别良恶性肺结节的AUC分别为0.725、0.745、0.627、0.613;不同类型非良性结节的K^(trans)、K_(ep)、V_(e)、V_(p)值对比无明显差异(P﹥0.05)。结论K^(trans)、K_(ep)值对良恶性肺结节具有诊断价值,且对肺鳞癌及肺腺癌具有鉴别价值。 Objective To explore the diagnostic value of dynamic contrast-enhanced MRI(DCE-MRI)in benign and malignant lung nodules not longer than 2cm.Methods A total of 101 patients with solitary lung nodules not longer than 2cm were enrolled as the research objects between March 2018 and February 2021.All underwent DCE-MRI examination and needle biopsy.The benign and malignant nodules,and their specific types were determined based on biopsy results.DCE-MRI parameters[volume transfer constant(K^(trans)),blood reflux constant(K_(ep)),extracellular extravascular volume fraction(V_(e)),contrast agent volume fraction in plasma(Vp)]of benign and malignant nodules were compared.And their differential value for benign and malignant nodules was analyzed.DCE-MRI parameters between lung adenocarcinoma and lung squamous cell carcinoma were compared,and their differential value for lung squamous cell carcinoma and lung adenocarcinoma was analyzed.DCE-MRI parameters in different types of benign nodules were compared.Results K^(trans),K_(ep),V_(e)and V_(p)values of malignant nodules were greater than those of benign nodules(P﹤0.05).AUC values of K^(trans),K_(ep),V_(e)and V_(p)in the differential diagnosis of benign and malignant lung nodules were 0.774,0.781,0.719 and 0.670,respectively.K^(trans)and K_(ep) values of lung squamous cell carcinoma were lower than those of lung adenocarcinoma(P﹤0.05).AUC values of K^(trans),K_(ep),V_(e)and V_(p) in the differential diagnosis of lung adenocarcinoma and lung squamous cell carcinoma were 0.725,0.745,0.627 and 0.613,respectively.There was no significant difference in K^(trans),K_(ep),V_(e) and V_(p) values among different types of benign lung nodules(P﹥0.05).Conclusion K^(trans)and K_(ep) values are of diagnostic value for benign and malignant lung nodules,and are of differential value for lung squamous cell carcinoma and lung adenocarcinoma.
