摘要
目的:探讨声辐射力脉冲弹性成像诊断干燥综合征(Sjogren Syndrome,SS)唾液腺病变的应用价值。方法:收集37例SS患者(SS组)和同期体检健康者40例(对照组),均实施声辐射力脉冲弹性成像检测下颌下腺、腮腺情况,记录剪切波速度,绘制受试者操作特征(Receiver Operating Characteristic,ROC)曲线,评估该技术诊断SS唾液腺病变的效能,获取SS的剪切波速度最佳截断值,并使用多因素回归分析剪切波速度与实验室指标的相关性。结果:SS组患者下颌下腺、腮腺的剪切波速度明显高于对照组,差异有统计学意义,P<0.05。SS患者腮腺的剪切波速度最佳截断值为2.14m/s,敏感度是94.69%,特异度是86.12%。下颌下腺剪切波速度最佳截断值为1.94m/s,敏感度为82.80%,特异度为86.75%。使用多因素回归分析,腮腺剪切波速度与类风湿因子、红细胞沉降率、免疫球蛋白G存在相关性,差异P<0.05。结论:声辐射力脉冲弹性成像技术诊断SS唾液腺病变应用价值高,通过测量腺体剪切波速度可反映疾病的活动情况,提高早期诊断率。
Objective:To explore the application value of acoustic radiation force pulse elastography in the diagnosis of SS salivary gland lesions.Methods:37 patients with SS(SS group)and 40 healthy subjects in the same period(control group)were collected.Acoustic radiation force pulse elastography was used to detect the situation of submandibular gland and parotid gland,record the shear wave velocity,draw the working characteristic(ROC)curve of subjects,evaluate the efficiency of this technology in diagnosing SS salivary gland disease,and obtain the best cutoff value of shear wave velocity of SS.The correlation between shear wave velocity and laboratory indexes was analyzed by multivariate regression.Results:The shear wave velocity of submandibular gland and parotid gland in SS group was significantly higher than that in the control group(P<0.05).The optimal threshold of shear wave velocity of parotid gland in SS patients was 2.14m/s,the sensitivity was 94.69%,and the specificity was 86.12%.The optimal threshold of submandibular gland shear wave velocity was 1.94m/s,the sensitivity was 82.80%,and the specificity was 86.75%.Multivariate regression analysis showed that the shear wave velocity of parotid gland was correlated with rheumatoid factor,erythrocyte sedimentation rate and immunoglobulin G(P<0.05).Conclusion:Acoustic radiation force pulse elastography has high application value in the diagnosis of SS salivary gland lesions.Measuring the shear wave velocity of glands can reflect the activity of the disease and improve the early diagnosis rate.
