双层探测器光谱CT鉴别诊断孤立性肺结节良恶性的价值 被引量:2

The Value of Dual-Layer Spectral Detector CT in Distinguishing Benign and Malignant Solitary Pulmonary Nodules

摘要 目的:探讨双层探测器光谱CT对孤立性肺结节(Solitary Pulmonary Nodule,SPN)良恶性的鉴别诊断价值。方法:回顾性分析2020年11月~2021年12月福建中医药大学附属第二人民医院的45例(28个恶性结节和17个良性结节)SPN患者的临床及光谱CT资料。患者均进行了动脉期和静脉期的胸部增强扫描。测量并统计出SPN两期的碘密度(Iodine Concentration,IC)、有效原子序数(Z-effective,Z_(eff))和标准化IC(Normalized Iodine Concentrations,NIC)。用独立样本t检验比较恶性结节和良性结节的光谱参数之间的差异。结果:恶性与良性SPN光谱CT参数比较,恶性SPN静脉期CT值、IC值、Z_(eff)值、NIC值(分别为75.70±13.53、1.96±0.66、0.39±0.05、8.33±0.38)低于良性SPN(89.78±14.98、2.42±0.68、0.43±0.04、8.64±0.46),二者之间统计比较有显著差异(P<0.05),其余参数统计分析无显著差异(P>0.05)。结论:双层探测器光谱CT能够提供IC值、Z_(eff)值等定量参数,对鉴别诊断良恶性SPN有重要临床价值。 Objective:To evaluate the differential diagnosis of benign and malignant solitary pulmonary nodules(SPN)by dual-layer spectral detector CT.Methods:Clinical and spectral CT data of 45 SPN patients(28 malignant and 17 benign nodules)in the Second Affiliated Hospital of Fujian University of Traditional Chinese Medicine from November 2020 to December 2021 were retrospectively analyzed.All patients underwent arterial and venous chest enhancement scans.The iodine concentration(IC),Z-effective(Z_(eff))and normalized iodine concentrations(NIC)of SPN were measured and calculated.The independentsamples test was used to compare the difference in spectral parameters between malignant and benign nodules.Results:Compared the spectral CT parameters of malignant SPN with benign SPN,the CT value,IC value,Zeff value and NIC value of malignant SPN in venous phase(75.70±13.53,1.96±0.66,0.39±0.05,8.33±0.38,respectively)were lower than those of benign SPN(89.78±14.98,2.42±0.68,0.43±0.04,8.64±0.46).There was a significant difference between the two(P<0.05),and there was no significant difference in other parameters(P>0.05).Conclusion:Double detector spectral CT can provide quantitative parameters such as IC value and Zeff value,which has important clinical value in differential diagnosis of benign and malignant SPN.
