摘要
目的:建立专科病种的标准化手术配包体系,提高手术相关工作效率。方法:基于医院近8年消毒供应配包数据和术中补充器械数据,采用自然语言处理(NLP)技术对手术单中非结构化数据进行实体提取,使用文本相似度算法映射构建标准的结构化数据完成手术配包的标准化定制;同时针对不同手术医生的使用习惯,结合手术名称、患者诊断等信息训练模型,基于相似度推荐或用户群体系统过滤推荐实现手术配包的个性化推荐。结果:医生在申请手术时完成手术名称和术式等信息填写,即可获取包含手术器械和材料明细的手术配包,并可根据患者情况再次调整物品数量。在胃癌、结直肠癌和骨科髋关节手术中的应用显示,使用标准化手术配包后,患者术后平均住院天数、耗材均次费用及手术时间均明显下降,术中二次药械配送次数明显减少。结论:基于大数据分析的标准化专科手术配包可以提高手术准备工作效率,降低运行成本,助力医院的高质量发展。
Objective To improve operation related work efficiency by establishing a standardized surgical kit system for specialized diseases. Methods Based on the hospital data regarding sterilized surgical kits and supplementary intraoperative instruments in last eight years, non-structured data entities in operation sheets were extracted by applying the natural language processing(NLP) technology, and standard structured data was mapped and constructed by applying text similarity algorithm, to achieve standardized customization of surgical kits. At the same time, given the use habits of different surgeons and combined with information training models including the operation name and patient diagnosis, we aim to achieve personalized recommendation of surgery kit based on similarity recommendation or user group system filtering recommendation. Results When the doctors completed the surgical application by submitting information of operation name and type, they could obtain a surgical kit containing surgical materials and instruments and a detailed list, and could also adjust the number of items again according to the patient’s condition. It showed that the average postoperative hospitalization days, average-time cost of surgical consumables and operation time were all significantly decreased, and the number of intraoperative secondary drug and equipment delivery was also significantly reduced, after using the standardized surgical kit in gastric cancer, colorectal cancer and articulatio coxae operations. Conclusion Standardized surgical kit based on big data analysis can improve the efficiency of surgical preparation, reduce the operation costs, and facilitate the high-quality development of hospitals.
