高血压对颈动脉斑块的影响及斑块特点与血管狭窄的关系 被引量:1

The effect of hypertension on carotid plaque and the relationship between plaque characteristics and vascular stenosis

摘要 目的:探讨高血压与颈动脉斑块的产生、分布、成分的关系,分析斑块成分及大小与血管狭窄的关系。方法:收集2019年1月—2021年12月间在南通市中医院行颈动脉CT血管造影(computed tomography angiography,CTA)125例患者的临床及影像资料,其中高血压组73例,检出颈动脉斑块120处;对照组52例,检出颈动脉斑块57处。分析两组病例在临床资料、斑块分布、类型及血管狭窄程度方面的差异,比较钙化斑块、脂肪斑块、混合斑块所致血管狭窄率的差异,分析斑块最大横截面积及体积与血管狭窄率的关系。结果:两组患者的年龄、脑梗死、糖尿病及颈动脉斑块构成比差异均有统计学意义(均P<0.05);性别、冠心病、高脂血症、斑块分布及类型、颈动脉狭窄程度构成比差异均无统计学意义(均P>0.05)。钙化斑块所致血管狭窄率最低,混合斑块所致血管狭窄率最高,钙化斑块与脂肪斑块所致血管狭窄率差异无统计学意义(P>0.05),钙化斑块与混合斑块、脂肪斑块与混合斑块所致血管狭窄率差异有统计学意义(P<0.05)。斑块最大横截面积及体积与血管狭窄率呈正相关(r=0.461、0.476,均P<0.001)。结论:高血压促进颈动脉斑块的形成,与钙化斑块相比,脂肪斑块及混合斑块更易引起血管狭窄,斑块最大横截面积及体积与血管狭窄率呈正相关。 Objective:To explore the relationship between hypertension and the generation,distribution,composition of carotid plaque,and analyze the relationship between the composition and size of plaque and vascular stenosis.Methods:The clinical and imaging data of 125 patients who underwent carotid artery computed tomography angiography(CTA)examination in the Traditional Chinese Medical Hospital of Nantong from January 2019 to December 2021 were retrospectively collected.They were divided into hypertension group(73 patients)and control group(52 patients).120 carotid plaques were detected in hypertension group,and 57 carotid plaques were detected in control group.The differences of clinical data,plaque distribution,type and degree of vascular stenosis between the two groups were analyzed.The differences of vascular stenosis rate caused by calcified plaque,fatty plaque and mixed plaque were analyzed too.The relationship between the maximum cross section area and volume of the plaque and the stenosis rate of the blood vessel were analyzed.Results:There were significant differences in age,whether or not with cerebral infarction,diabetes and carotid plaque between the hypertension group and the control group(P<0.05).There was no statistical difference in gender,whether or not with coronary heart disease and hyperlipidemia,plaque distribution and type,composition ratio of carotid artery stenosis degree between the two groups(P>0.05).The vascular stenosis rate caused by calcified plaque was the lowest,the vascular stenosis rate caused by mixed plaque was the highest.There was no statistical difference in the vascular stenosis rate caused by calcified plaque and fatty plaque(P>0.05).The vascular stenosis rates caused by calcified plaque and mixed plaque,fatty plaque and mixed plaque were statistically different(P<0.05).The maximum cross-sectional area and volume of plaque had a positive correlation with the vascular stenosis rate(r=0.461,0.476,P<0.001).Conclusion:Hypertension promotes the formation of carotid plaques.Compared wit
