摘要
目的探讨产前超声检查对胎儿脐-门-体静脉异常分流(umbilical-portal-systemic venous shunts,UPSVS)的诊断价值。方法选取2017年1月~2021年1月四川省妇幼保健院产前超声诊断的24例胎儿UPSVS声像图特征及其随访结果,按照最新UPSVS超声分型方法进行分型,探讨各型的临床意义。结果24例UPSVS胎儿中,脐静脉-体静脉分流(umbilical-systemic shunts,USS)5例,静脉导管-体静脉分流(ductus venosus-systemic shunts,DVSS)5例,肝内门静脉-体静脉分流(intrahepatic portal-systemic shunts,IHPSS)13例,肝外门静脉-体静脉分流(extrahepatic portal-systemic shunts,EHPSS)1例。结论产前超声检查对胎儿UPSVS的早期诊断具有重要临床价值,准确分型可对临床优生咨询及产后治疗具有重要指导意义。
Objective To explore the diagnostic value of prenatal ultrasound in fetal umbilical-portal-systemic venous shunt.Methods A retrospective study was performed in 24 cases with fetal UPSVS,including the ultrasonic characteristics and follow-up results,which were diagnosed by the department of ultrasound in sichuan maternal and child health hospital from January 2017 to January 2021.Identified cases of UPSVS were analyzed according to the new classification.Results Among the 24 UPSVS fetuses,there were 5 with USS,5 with DVSS,13 with IHPSS and 1 with EHPSS.Conclusion Prenatal ultrasound is of great clinical value in the early diagnosis of fetal UPSVS.Accurate classification can provide important guidance for clinical eugenic consultation and postpartum treatment.
