摘要
特发性膜性肾病(Idiopathic Membranous Nephropathy,IMN)是成人原发性肾小球疾病中常见的病理类型之一,患者以中老年人多见,40岁以上为发病高峰期。多数患者表现为肾病综合征,出现不同程度的水肿、低蛋白血症、高脂血症和蛋白尿等症状。西医治疗以激素联合免疫抑制剂为主,疗效欠佳且不良反应较大。中医缓解临床症状的作用明显,不良反应小,受到广泛关注。熊维建主任从事中医治疗肾病工作多年,具有丰富的临床经验。其认为本病以脾肾气虚,精微外泄为本,湿瘀互结为其标,风邪扰动为其患。治疗上多运用参芪地黄汤益气健脾,滋肾固精以治本;祛湿利水,活血化瘀以治标;从风论治,常合用昆仙胶囊除其患。这一方法获得了不错的疗效。文章通过案例总结熊维建主任运用参芪地黄汤联合昆仙胶囊治疗IMN的经验,希望为临床提供参考。
Idiopathic membranous nephropathy(IMN)is one of the common pathological types of Nprimary glomerular disease in adults.Middle and elderly people are more common,over 40 years old for the peak.Most patients present with phrotic syndrome,with varying degrees of edema,hypoproteinemia,hyperlipidemia,and proteinuria.Western medicine mainly uses hormone combined with immunosuppressant,with poor curative effect and adverse reactions.TCM has received widespread attention because of its obvious effect of relieving clinical symptoms,and few adverse reactions.Director XIONG Weijian has been engaged in the treatment of nephropathy in TCM for many years,and has rich clinical experience.The Shenqi Dihuang decoction(参芪地黄汤)and the Kunxian capsule(昆仙胶囊)was used to eliminate the disease,and has obtained a good effect.This article summarizes Director XIONG Weijian’s clinical experience in treating idiopathic membrane nephropathy with the Shenqi Dihuang decoction and the Kunxian capsules,hoping to provide clinical reference.
