摘要
目的:分析苓甘五味姜辛汤合止嗽散加减治疗老年慢性咳嗽的临床疗效。方法:选取2017年1月1日-2019年12月31日华中科技大学协和深圳医院收治的老年慢性咳嗽患者90例,并采用电脑数字随机分组的方法将其分为对照组和观察组,每组45例。对照组患者给予常规的西医治疗法进行治疗,观察组给予苓甘五味姜辛汤合止嗽散加减治疗。对比两组患者治疗后的咳嗽症状减轻时间及咳嗽消失时间、中医证候积分、小气道功能、血生化指标及不良反应发生情况。结果:观察组患者的咳嗽症状减轻时间及咳嗽消失时间均短于对照组患者(P<0.05);观察组患者的中医证候积分中的咳嗽、痰量、神疲乏力及气短胸闷评分均低于对照组患者(P<0.05);观察组治疗后的最大呼气流量(Peak Expiratory Flow,PEF)、用力呼气流速(Forced Expiratory Flow,FEF)、呼气中期流速(Maximum Mid-expiratory Flow,MMEF)等小气道功能指标高于对照组,呼出气一氧化氮(Fractional Exhaled Nitric Oxide,FeNO)、食管24 h酸碱值(pH)、血清总免疫球蛋白E(Immunoglobulin E,IgE)低于对照组(P<0.05),且观察组患者的总不良反应发生率低于对照组患者(P<0.05)。结论:采用苓甘五味姜辛汤合止嗽散加减治疗老年慢性咳嗽能显著改善老年慢性咳嗽患者的临床症状、体征,对改善患者的小气道功能、血生化指标均有积极意义,且安全性高,治疗效果更加显著。
Objective:To analyze efficacy of the Linggan Wuwei Jiangxin decoction(苓甘五味姜辛汤)plus Zhisou San(止嗽散)on senile chronic cough.Methods:Fron January 1,2017 to December 31,2019,a total of 90 cases were divdied into 2 groups,with 45 cases in each.The control group was given conventional Western medicine,and the observation group was given the Linggan Wuwei Jiangxin decoction plus Zhisou San.Results:The cough symptom relief time and cough disappearance time in the observation group was shorter than the control group(P<0.05);the TCM syndrome scores of cough,phlegm,fatigue,shortness of breath and chest tightness in the observation group were lower than those in the control group(P<0.05);the small airway function indexes such as peak expiratory flow(PEF),forced expiratory flow(FEF),and maximum mid-expiratory flow(MMEF)in the observation group after treatment were higher than those in the control group,and the exhaled nitric oxide(Fractional Exhaled Nitric Oxide,FeNO),esophagus 24 h pH value,serum total immunoglobulin E in the control group were lower than those in the control group(P<0.05).The incidence of total adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusion:The Linggan Wuwei Jiangxin decoction plus Zhisou San on senile chronic cough can ameliorate clinical symptoms and signs,small airway function,blood biochemical indexes,and has high safety.
