摘要
目的探讨责任制整体护理联合多元化教育对瘢痕子宫妊娠再次剖宫产产妇负性情绪及妊娠结局的影响。方法选取2019年1月-2021年8月我院收治的瘢痕子宫妊娠行再次剖宫产的产妇76例。以随机数字表法将分为妊娠A组和妊娠B组,每组38例。妊娠A组在围产期护理干预中给予常规护理联合多元化教育,妊娠B组给予责任制整体护理联合多元化教育。比较两组产妇干预前后负性情绪改善情况、妊娠结局、产后24h VAS评分、产后48h血清泌乳素(PRL)水平、产后至首次母乳喂养时间,以及对围产期护理干预的总体评价。结果两组干预后负性情绪评分低于干预前,妊娠B组低于妊娠A组(P<0.05);妊娠B组不良妊娠结局发生率低于妊娠A组,产后24h VAS评分低于妊娠A组,产后48h PRL水平高于妊娠A组,产后至首次母乳喂养时间短于妊娠A组,总体评价高于妊娠A组(P<0.05)。结论行二次剖宫产分娩的瘢痕子宫妊娠产妇采取责任制整体护理联合多元化教育干预模式,有利于减轻产妇负性情绪,减少不良妊娠结局,改善预后,且该护理干预模式获得产妇更高的总体评价。
Objective To explore the effect of responsibility system holistic nursing combined with diversified education on negative emotion and pregnancy outcome of re-cesarean section women with scarred uterus pregnancy.Methods A total of 76 pregnant women with scarred uterus who underwent cesarean section again in our hospital from January 2019 to August 2021 were selected.They were divided into pregnancy group A and pregnancy group B by random number table method,with 38 cases in each group.Pregnancy group A was given routine nursing combined with diversified education in perinatal nursing intervention,and pregnancy group B was given responsibility system holistic nursing combined with diversified education.The improvement of negative emotion,pregnancy outcome,VAS score at 24 h postpartum,serum prolactin(PRL)level at 48 h postpartum,time from postpartum to first breastfeeding,and overall evaluation of perinatal nursing intervention were compared between the two groups.Results The negative emotion scores of the two groups after intervention were lower than those before intervention,and the pregnancy group B was lower than the pregnancy group A(P<0.05).The incidence of adverse pregnancy outcomes in group B was lower than that in group A,the VAS score at 24 h after delivery was lower than that in pregnancy group A,the PRL level at 48 h after delivery was higher than that in pregnancy group A,and the time from delivery to the first breastfeeding was shorter than that in pregnancy group A(P<0.05).The overall evaluation of pregnancy group B was higher than that of pregnancy group A(P<0.05).Conclusion The responsibility system holistic nursing combined with diversified education intervention model is conducive to reducing maternal negative emotions,reducing adverse pregnancy outcomes and improving prognosis in pregnant women with scar uterus undergoing secondary cesarean section,and the nursing intervention model has a higher overall evaluation.
