摘要
目的:调查非静脉曲张性上消化道出血病人非计划再入院的现况,并分析其影响因素。方法:采用病例对照研究方法对2019年5月—2021年5月在山西省3所三级甲等医院首次住院的523例因非静脉曲张性上消化道出血入院病人的非计划再入院情况进行调查,并分析其影响因素。结果:42例病人发生非计划再入院,发生率为8.03%;多因素Logistic回归分析显示,年龄(OR=1.071)、查尔森合并症指数(CCI)(OR=2.718)、首次住院时长(OR=1.446)、格拉斯哥⁃布拉奇福德出血评分(GBS)评分(OR=1.338)、是否服用非甾体类抗炎药(OR=3.118)、饮酒史(OR=3.445)、是否进行内镜检查(OR=0.350)、是否输血(OR=4.061)是非静脉曲张性上消化道出血病人非计划再入院的影响因素(P<0.05)。结论:非静脉曲张性上消化道出血病人非计划再入院率较高,其发生受病人人口社会学因素、疾病因素和环境因素等多方面的影响。提示护理人员应有效识别发生非计划再入院的危险因素,有针对性地采取个性化护理措施及健康教育,以降低再入院率。
Objective:To investigate the status and influencing factors of unplanned readmission in patients with non⁃varicose upper gastrointestinal bleeding.Methods:A case⁃control study was conducted to investigate the unplanned readmission of 523 patients hospitalized for non⁃varicose upper gastrointestinal bleeding in three tertiary grade A hospitals in Shanxi province from May 2019 to May 2021,and analyze the influencing factors.Results:Unplanned readmission occurred in 42 patients(8.03%).Multivariate regression analysis showed that age(OR=1.071),CCI index(OR=2.718),length of first hospital stay(OR=1.446),GBS score(OR=1.338),non⁃steroidal anti⁃inflammatory drugs(OR=3.118),historyof drinking(OR=3.445),endoscopy(OR=0.350),blood transfusion(OR=4.061)were the influencing fac to rs of unplanned readmission in pa tien ts with non⁃va ricose uppe r gas troin tes tinal bleeding(P<0.05).C o n clu sio n:T he unplanned readmission rate of non⁃varicose upper gastrointestinal bleeding is high,and its occurrence is influenced by demographic and social factors,disease factors and environmental factors.It is suggested that nursing staff should effectively identify the risk factors of unplanned readmission and take personalized nursing measures and health education to reduce the readmission rate.
