人工泪液对白内障患者角膜测量的影响研究

摘要 目的:探讨人工泪液对白内障患者角膜测量的影响。方法:前瞻性选择2021年4月至9月在南昌爱尔眼科医院行白内障手术治疗的100例患者为研究对象,按是否合并干眼症分为两组,50例无干眼综合征的正常眼患者纳入对照组,50例轻度至中度干眼综合征患者纳入干眼组,所有患者检查前均使用0.1%玻璃酸钠滴眼液(低浓度)、0.3%玻璃酸钠滴眼液(高浓度)滴眼。使用IOL Master 700(Carl Zeiss Meditec AG,德国)测量各组滴眼液前与滴不同浓度眼液后2 min、5 min和10 min的中央角膜厚度(CCT)、眼轴长度(AL)、平坦轴角膜曲率(K1)、陡峭轴角膜曲率(K2),并计算人工晶状体(IOL)度数。对比对照组与干眼组各项测量结果,并分析不同浓度玻璃酸钠滴眼液对干眼患者各项测量结果的影响。结果:对照组滴眼液后2 min与5 min时CCT与AL均大于滴眼液前,IOL度数小于滴眼液前,差异有统计学意义(P<0.05);干眼组滴眼液前的K1、K2与滴眼液后比较,差异无统计学意义(P>0.05)。干眼组滴眼液后2 min与5 min时 CCT与AL均大于滴眼液前,IOL度数、K1度数小于滴眼液前,且滴眼液后5 min时K2度数小于滴眼液前,差异有统计学意义(P<0.05)。滴眼液前干眼组CCT、K1、K2均大于对照组,IOL小于对照组,差异有统计学意义(P<0.05),两组AL比较,差异无统计学意义(P>0.05);滴眼液后2 min与5 min时干眼组CCT、IOL度数低于对照组(P<0.05),滴眼液后10 min时两组CCT、IOL度数差异无统计学意义(P>0.05);干眼患者滴眼液前低浓度组与高浓度组各项测量指标对比,差异无统计学意义(P>0.05);高浓度组滴眼液后2 min与5 min、10 min时 IOL度数小于低浓度组,AL、CCT、K1、 K2度数大于低浓度组,差异有统计学意义(P<0.05)。结论:人工泪液可直接影响白内障患者角膜测量结果,滴眼液的浓度越高,对角膜的影响就越大,测量之前所需的间隔时间可能越长。 Objective:To investigate the effect of artificial tears on corneal measurement in cataract patients.Methods:100 patients who underwent cataract surgery in Nanchang Aier Eye Hospital from April to September 2021 were prospectively selected as the research objects.They were divided into two groups according to whether they were combined with dry eye syndrome.50 normal eye patients without dry eye syndrome were included in the control group,and 50 patients with mild to moderate dry eye syndrome were included in the dry eye group.All patients were treated with 0.1% sodium hyaluronate eye drops (low concentration) and 0.3% sodium hyaluronate eye drops (high concentration) before examination.IOL master 700 (Carl Zeiss meditec AG,Germany) was used to measure the central corneal thickness (CCT),axial length (AL),horizontal axial corneal curvature (K1),and steep axial corneal curvature (K2) before and 2,5,and 10 minutes after eye drops of different concentrations in each group,and the degree of intraocular lens (IOL) was calculated.The measurement results of the control group and the dry eye group were compared,and the effects of different concentrations of sodium hyaluronate eye solution on the measurement results of dry eye patients were analyzed.Results:In the control group,CCT and Al were greater than those before eye drops at 2 and 5 minutes after eye drops,and the degree of IOL was less than that before eye drops,the difference was statistically significant (P0.05).In the dry eye group,the CCT and Al were greater than those before the eye drops at 2 min and 5 min after the eye drops,the IOL degree and K1 degree were less than those before the eye drops,and the K2 degree was less than those before the eye drops at 5 min after the eye drops,the difference was statistically significant (P0.05);The CCT and IOL degrees in the dry eye group were lower than those in the control group at 2 min and 5 min after eye drops,and the difference was statistically significant (P0.05);there was no significant difference between the l
