摘要
目的分析多导睡眠监测仪(PSG)对急性缺血性脑卒中的临床价值。方法选取101例急性缺血性脑卒中患者作为研究对象,采用匹兹堡睡眠质量指数量表(PSQI)评估患者失眠症状,基于PSQI评分分为失眠组(PSQI评分≥8分,55例)和非失眠组(PSQI评分<8分,46例),应用PSG对两组患者进行睡眠监测,观察比较两组患者基本人口学资料及PSQI评分,PSG相关睡眠参数[睡眠总时间、睡眠潜伏期、快速动眼睡眠(REM)潜伏期、REM期、觉醒次数];并分析急性缺血性脑卒中患者PSQI评分与PSG相关睡眠参数的相关性。结果两组患者的性别、年龄比较差异均无统计学意义(P>0.05);失眠组患者的PSQI评分(14.23±4.11)分高于非失眠组的(5.32±2.94)分,差异有统计学意义(P<0.05)。两组患者REM潜伏期比较,差异无统计学意义(P>0.05);失眠组睡眠总时间[291(214,357)]min、REM期[52(29,74)]min均短于非失眠组的[380(314,478)]、[86(65,111)]min,睡眠潜伏期[58(36,72)]min长于非失眠组的[21(13,34)]min,觉醒次数[5(3,7)]次多于非失眠组的[2(0,4)]次,差异均有统计学意义(P<0.05)。急性缺血性脑卒中患者的PSQI评分与睡眠总时间、REM期呈负相关(r=-0.559、-0.268,P<0.05),与睡眠潜伏期呈正相关(r=0.375,P<0.05),与REM潜伏期、觉醒次数无相关性(P>0.05)。结论通过PSG可以对急性缺血性脑卒中患者的失眠进行准确评估,睡眠总时间、睡眠潜伏期、REM期能够对急性缺血性脑卒中患者的失眠程度进行有效的反映。
Objective To analyze the clinical value of polysomnography(PSG)in acute ischemic stroke.Methods 101 acute ischemic stroke patients were selected as the study subjects,and the Pittsburgh Sleep Quality Index Inventory(PSQI)was used to assess the patients'insomnia symptoms.Based on the PSQI score,the patients were divided into insomnia group(PSQI score≥8 points,55 cases)and non-insomnia group(PSQI score<8 points,46 cases),and the PSG was applied to monitor the sleep of patients in the two groups.The basic demographic data and PSQI score,PSG-related sleep parameters[total sleep time,sleep latency,rapid eye movement(REM)latency,REM phase,number of awakenings]were observed and compared between the two groups;and the correlation between PSQI score and PSG-related sleep parameters in patients with acute ischemic stroke was analyzed.Results There was no statistically significant difference in gender and age between the two groups(P>0.05).The PSQI score(14.23±4.11)points in the insomnia group was higher than(5.32±2.94)points of the non-insomnia group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in REM latency between the two groups(P>0.05).The total sleep time[291(214,357)]min and REM phase[52(29,74)]min in the insomnia group were shorter than[380(314,478)]and[86(65,111)]min in the non-insomnia group;the sleep latency[58(36,72)]min in the insomnia group was longer than[21(13,34)]min in the non-insomnia group;the number of awakenings[5(3,7)]times in the insomnia group was more than[2(0,4)]times in the non-insomnia group;the differences were all statistically significant(P<0.05).PSQI score of acute ischemic stroke patients was negatively correlated with total sleep time and REM phase(r=-0.559,-0.268;P<0.05),positively correlated with sleep latency(r=0.375,P<0.05),and not correlated with REM latency or number of awakenings(P>0.05).Conclusion PSG can accurately assess the insomnia in patients with acute ischemic stroke.Total sleep time,sleep latency,and REM phas
