摘要
目的探讨角膜溃疡患者实施多层生物羊膜移植术联合绷带式角膜接触镜治疗的效果。方法52例角膜溃疡患者,随机分为对照组和观察组,每组26例(26眼)。对照组单用多层生物羊膜移植术治疗,观察组实施多层生物羊膜移植术联合绷带式角膜接触镜治疗。比较两组的临床疗效、并发症发生情况、术后恢复指标、术后角膜恢复情况及手术前后视力。结果观察组治疗总有效率为92.31%,明显高于对照组的69.23%,差异具有统计学意义(P<0.05)。术后1、3、6个月,观察组视力分别为(0.47±0.12)、(0.59±0.14)、(0.70±0.15),明显高于对照组的(0.32±0.11)、(0.40±0.13)、(0.45±0.12),差异具有统计学意义(P<0.05)。观察组角膜上皮愈合时间(14.52±5.62)d、角膜炎症治愈时间(14.09±6.28)d明显短于对照组的(22.17±8.04)、(20.97±8.42)d,术后6个月角膜散光度(-3.65±1.49)D低于对照组的(-5.77±1.92)D,差异具有统计学意义(P<0.05)。两组并发症发生率比较差异无统计学意义(P>0.05)。两组患眼术后3、6个月角膜内皮细胞密度均低于本组健眼,但观察组角膜内皮细胞密度(2047±136)、(2025±132)个/mm2高于对照组的(1902±131)、(1876±125)个/mm2,差异具有统计学意义(P<0.05)。结论角膜溃疡患者实施多层生物羊膜移植术联合绷带式角膜接触镜治疗的效果显著,能有效修复受损角膜,提高视力,且并发症发生率低,预后好。
Objective To discuss the effect of multi-layer amniotic membrane transplantation combined with bandage contact lens in patients with corneal ulcer.Methods A total of 52 cases of corneal ulcer patients were randomly divided into control group and observation group,with 26 cases(26 eyes)in each group.The control group was treated with multi-layer amniotic membrane transplantation alone,and the observation group was treated with multi-layer amniotic membrane transplantation combined with bandage contact lens.The clinical efficacy,incidence of complications,postoperative recovery indicators,postoperative corneal recovery,and visual acuity before and after surgery were compared between the two groups.Results The total effective rate of the observation group was 92.31%,which was significantly higher than 69.23%of the control group,and the difference was statistically significant(P<0.05).At 1,3,and 6 months after surgery,the visual acuity of the observation group were(0.47±0.12),(0.59±0.14),and(0.70±0.15),which were significantly higher than(0.32±0.11),(0.40±0.13),and(0.45±0.12)of the control group,and the differences were all statistically significant(P<0.05).The corneal epithelial healing time(14.52±5.62)d and the corneal inflammation healing time(14.09±6.28)d in the observation group were significantly shorter than(22.17±8.04)and(20.97±8.42)d in the control group;the corneal astigmatism 6 months after surgery(-3.65±1.49)D in the observation group was lower than(-5.77±1.92)D in the control group;the differences were all statistically significant(P<0.05).There was no statistically significant difference in the incidence of complications between the two groups(P>0.05).The corneal endothelial cell density of affected eyes at 3 and 6 months after surgery was lower in both groups than that in the healthy eyes of this group,but the corneal endothelial cell density in the observation group[(2047±136)and(2025±132)cells/mm2]were higher than those in the control group[(1902±131)and(1876±125)cells/mm2],and the di
