摘要
目的评估在胸部外照射放疗的基础上使用气管腔内放疗治疗中晚期非小细胞肺癌的效果和不良反应。方法收集南京市胸科医院2016年7月至2019年12月收治的60例Ⅲa~Ⅳ期非小细胞肺癌患者作为研究对象,根据随机数字表法将其分为试验组和对照组,其中试验组29例采取气管腔内放疗+外照射方案(气管腔内放疗DT 10 Gy/2 f+外照射DT 55~56 Gy),对照组31例采用常规胸部外照射调强放疗方案DT 60 Gy/30 f。治疗后比较两组近期疗效、不良反应发生率及1年生存率。结果放疗后1个月两组客观缓解率比较,差异无统计学意义(P>0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。随访12个月,试验组1年生存率高于对照组(P<0.05)。结论对于局部中晚期非小细胞肺癌患者,气管腔内放疗联合外照射降低胸部外照射剂量的同时可稳定近期疗效,有助于减少放射性肺炎、放射性食管炎等不良反应的发生率,且1年生存率较高。
Objective To evaluate the efficacy and adverse events of intratracheal radiotherapy combined with external irradiation in the treatment of advanced non-small cell lung cancer.Methods A total of 60 patients with stageⅢa toⅣnon-small cell lung cancer treated in Nanjing Chest Hospital from July 2016 to December 2019 were selected as the research objects.According to the random number table method,they were divided into experimental group and control group.A total of 29 patients in the experimental group were treated with endotracheal radiotherapy+external irradiation(endotracheal radiotherapy DT 10 Gy/2 f+external irradiation DT 55-56 Gy),and 31 patients in the control group were treated with conventional external chest irradiation intensity modulated radiotherapy DT 60 Gy/30 f.After treatment,the short-term efficacy,1-year local control rate and the incidence of adverse reactions were compared between the two groups.Results There was no significant difference in objective remission rate between the two groups at one month after radiotherapy(P>0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).The 1-year survival rate of the experimental group was higher than that of the control group after 12 months follow-up(P<0.05).Conclusion For patients with locally advanced non-small cell lung cancer,endotracheal radiotherapy combined with external irradiation can stabilize the short-term efficacy while reducing the dose of external chest irradiation,help to reduce the incidence of adverse reactions such as radiation pneumonia and radiation esophagitis,and the 1-year survival rate is high.
