摘要
目的:探讨冠心病合并2型糖尿病患者血糖、血脂指标变化情况及其临床意义。方法:102例冠心病合并2型糖尿病患者作为CDM组,同期入院的单纯冠心病患者作为CHD组。比较两组冠状动脉病变程度、血糖、血脂指标、血压及胆红素水平。结果:CDM组冠状动脉病变支数显著多于CHD组;CDM组FPG、HbA1c均显著高于CHD组;CDM组患者血清TC、TG、LDL-C、VLDL均高于CHD组,HDL-C显著低于CHD组;两组SBP、DBP、总胆红素(TB)、直接胆红素(DB)、间接胆红素(IB)差异均无统计学意义。Logistic回归分析提示,高HbA1c水平,低TB水平为冠心病合并2型糖尿病发生的独立危险因素。结论:冠心病合并2型糖尿病患者血糖、血脂水平较单纯冠心病患者升高,高HbA1c水平,低TB水平为冠心病合并2型糖尿病发生的独立危险因素。
Objective To investigate the changes of blood glucose,blood lipid indexes in patients with coronary heart disease complicated with type 2 diabetes mellitus and their clinical significance.Methods 102 patients with coronary heart disease and type 2 diabetes mellitus were selected as the CDM group,and patients with simple coronary heart disease who were admitted to the hospital during the same period were selected as the CHD group.The degree of coronary artery lesions,blood glucose,blood lipid indexes,blood pressure and bilirubin levels were compared between the two groups.Results The number of coronary lesions in the CDM group was significantly higher than that in the CHD group.The FPG and HbA1c in the CDM group were significantly higher than those in the CHD group.Serum TC,TG,LDL-C,and VLDL in CDM group were higher than those in CHD group,and HDL-C was significantly lower than that in CHD group.There was no significant difference in SBP,DBP,total bilirubin (TB),direct bilirubin (DB) and indirect bilirubin (IB) between the two groups.Logistic regression analysis showed that high HbA1c level and low TB level were independent risk factors for coronary heart disease complicated with type 2 diabetes.Conclusion The blood glucose and blood lipid levels in patients with coronary heart disease and type 2 diabetes are higher than those in patients with simple coronary heart disease.High HbA1c levels and low TB levels are independent risk factors for coronary heart disease and type 2 diabetes.
