摘要
腹腔镜肝切除术中对于断肝平面的把握十分重要,是决定手术成败的关键。从最初的术者经验、寻找解剖标记物和遵循缺血线的方法,到精密的术前规划、术中超声和荧光影像的使用,逐步提高了腹腔镜肝切除断肝平面的准确度。在未来,外科手术导航将克服气腹压、呼吸、心跳、手术操作等因素所致的肝脏变形引起的图像配准精确度下降的问题,通过人工智能导航系统的研发,进一步提高图像配准的精确度,保障腹腔镜肝切除手术的精准实施,前景可期。
Laparoscopic liver resection is very important for grasping the plane of liver fracture,which is the key to determine the success or failure of the operation.From initial operator experience,methods of finding anatomical landmarks and following ischemic lines,to precise preoperative planning,and the use of intraoperative ultrasound and fluoroscopic imaging,the accuracy of laparoscopic hepatectomy planes has been gradually improved.In the future,surgical navigation will overcome the problem of decreased image registration accuracy caused by liver deformation,which is caused by factors such as pneumoperitoneal pressure,respiration,heartbeat,surgical operation and other factors.Through the research and development of artificial intelligence navigation system,the accuracy of image registration can be further improved,and the accurate implementation of laparoscopic liver resection surgery can be guaranteed,with a promising prospect.
