摘要
目的:探讨分级诊疗模式下的糖尿病视网膜病变防治效果。方法:于2020年12月-2021年6月对抚州市宜黄县和崇仁县进行整体抽样筛选糖尿病患者1430例,按诊疗模式将其分为观察组(n=746)和对照组(n=684)。对照组以传统模式进行治疗及随访,观察组以分级诊疗模式进行治疗及随访。6个月后,比较两组糖尿病控制情况、糖尿病视网膜病变(DR)发病和治疗情况及医疗资源配置情况。结果:诊疗3个月及6个月,两组空腹血糖(FBG)、餐后2 h血糖(2 h PBG)较治疗前均有降低,且观察组均低于对照组(P<0.05);6个月内两组DR发病率比较,差异无统计学意义(P>0.05),但观察组DR分期情况优于对照组(P<0.05),观察组DR患者治疗总有效率高于对照组DR患者(P<0.05);观察组平均住院日短于对照组(P<0.05),总医疗费用低于对照组(P<0.05)。结论:分级诊疗模式能有效保证糖尿病患者对血糖水平的良好控制,提升DR的防治效果,降低医疗费用。
Objective:To explore the control effect of hierarchical diagnosis and treatment mode on diabetic retinopathy(DR).Method:According to overall sampling method,1430 patients with diabetes mellitus in Yihuang County and Chongren County,Fuzhou,were enrolled between December 2020 and June 2021.According to different diagnosis and treatment modes,they were divided into observation group(n=746)and control group(n=684).The control group was given traditional mode,while observation group was given hierarchical diagnosis and treatment mode for treatment and follow-up.After 6 months,control of diabetes mellitus,onset and treatment of DR,and allocation of medical resources were compared between the two groups.Result:At 3 and 6 months of diagnosis and treatment,fasting blood glucose(FBG)and 2 h postprandial blood glucose(2 h PBG)in both groups were decreased,which were lower in observation group than control group(P<0.05).There was no significant difference in incidence of DR within 6 months between the two groups(P>0.05),DR staging in observation group was better than that in control group(P<0.05),total effect rate of patients with DR in observation group was higher than that of patients with DR in control group(P<0.05);average hospitalization time was shorter than that in control group(P<0.05),and total medical cost was lower than that in control group(P<0.05).Conclusion:The hierarchical diagnosis and treatment mode can better control blood glucose,improve control effect of DR and reduce medical cost in patients with diabetes mellitus.
