摘要
目的:研究经压力微导管测量冠状动脉介入术中血流储备分数(FFR)的应用。方法:研究对象为九江市第一人民医院2020年6月-2021年6月行冠状动脉造影(CAG)并行冠状动脉介入治疗(PCI)病例60例,依据随机数字表法分两组,观察组30例每处病变均进行FFR测定,对FFR≤0.8处进行支架置入,对照组30例常规冠状动脉造影检查后行PCI治疗。对比两组术后平均支架植入数量、造影剂使用量、手术时间、病变数目及住院费用等,术后对两组随访半年,比较两组不良心血管事件发生情况。分析术后FFR值对不良心血管事件预测价值。结果:观察组平均支架植入数量、造影剂使用量,以及手术时间、住院费用均少于对照组(P<0.05);两组病变数量比较,差异无统计学意义(P>0.05);术后半年,观察组不良心血管事件发生率为6.67%,对照组为33.33%,观察组低于对照组,差异有统计学意义(P<0.05);术后FFR=0.868为不良心血管事件预测的最佳临界值。结论:经压力微导管测量FFR值大大提高并规范了冠状动脉介入诊疗,最大限度减少平均支架植入数量,减少住院费用,降低了心血管主要不良事件的发生率,具有较高应用价值。
Objective:To study the application of pressure microcatheter in measuring fractional flow reserve(FFR)during coronary intervention.Method:The research subjects were 60 patients who underwent coronary angiography(CAG)and percutaneous coronary intervention(PCI)in Jiujiang NO.1 People’s Hospital from June 2020 to June 2021.They were divided into two groups according to the random number table method.30 cases of each lesion were measured by FFR,stent placement was performed for the FFR≤0.8,and 30 cases of the control group were treated with PCI after routine coronary angiography.The average number of stents implanted,the amount of contrast agent used,the operation time,the number of lesions,and hospitalization costs were compared between the two groups.Postoperative FFR for predicting adverse cardiovascular events was analyzed.Result:The average number of stents implanted,the amount of contrast agent used,as well as the operation time and hospitalization costs in the observation group were less than those in the control group(P<0.05);there was no significant difference in the number of lesions between the two groups(P>0.05);postoperative in half a year,the incidence of adverse cardiovascular events in the observation group was 6.67%and that in the control group was 33.33%,the observation group was lower than that of the control group,the difference was statistically significant(P<0.05).Postoperative FFR=0.868 was the best critical value for predicting adverse cardiovascular events.Conclusion:The FFR value measured by pressure microcatheter greatly improves and standardizes the diagnosis and treatment of coronary intervention,minimizes the average number of stent implantation,reduces hospitalization costs,and reduces the incidence of major cardiovascular adverse events,which has high application value.
