关节镜下单针套扎法结合经胫骨拉线修复技术治疗内侧半月板后根部损伤 被引量:2

摘要 目的 探讨关节镜下单针套扎法结合经胫骨拉线修复技术治疗内侧半月板后根部损伤的临床效果。方法采用关节镜下单针套扎法结合经胫骨拉线修复技术治疗34例内侧半月板后根部损伤患者。记录并发症发生情况及膝关节活动度,根据IKDC评分、Lysholm评分、Kellgren-Lawrence分级评价疗效。结果 患者均获得随访,时间12~24个月。切口甲级愈合,无下肢深静脉血栓形成及切口延迟愈合、不愈合情况。IKDC评分、Lysholm评分术后3、6、12个月均高于术前(P<0.05);Kellgren-Lawrence分级手术前后比较差异均无统计学意义(P> 0.05)。末次随访时,34例膝关节屈曲110°~135°,伸直0°~5°。结论 关节镜下单针套扎法结合经胫骨拉线修复技术治疗内侧半月板后根部损伤并发症少,膝关节功能恢复良好。 Objective To investigate the clinical effect of single-needle entrapment combined with transtibial pullout suture repair technique for the posterior medial meniscus root tear(PMMRT).Methods The 34 patients with PMMRT were treated by single-needle entrapment combined with transtibial pullout suture repair technique. Complications and knee range of motion(ROM) were recorded,the curative effect was evaluated according to the International Knee Documentation Committee(IKDC) score,the Lysholm score and the Kellgren-Lawrence grade.Results All patients were followed up for 12 ~ 24 months. The incisions were Grade A healing,there were no lower limbs deep venous thrombosis,delayed healing and nonunion of incision after surgery. At 3,6,12 months,the IKDC scores and Lysholm scores were significantly higher than the postoperation(P< 0. 05);Kellgren-Lawrence grade had no significant difference before and after surgery(P> 0. 05). At the last follow-up,the knee ROM of 34 cases were flexion with 110° ~135° and extension 0° ~ 5°.Conclusions Arthroscopic single-needle entrapment combined with transtibial pullout suture repair technique has fewer complications for the treatment of PMMRT,with good recovery of knee function.
