双重任务训练对轻度认知障碍患者平衡功能的效果观察 被引量:2

摘要 目的探讨步行—认知双重任务训练对于改善轻度认知障碍患者平衡及认知能力的效果。方法选取2020年12月—2021年8月期间上海市第三康复医院收治的94例轻度认知障碍患者。采用数字表法随机分为对照组(n=46)和观察组(n=48)。对照组实施常规步行训练;观察组在常规步行训练的基础上增加认知功能训练,干预期为6个月。2组患者干预前后用Berg平衡量表(Berg Balance Scale,BBS)、蒙特利尔认知评估量表(Montreal cognitive assessment,MoCA)、计时—起立行走测试(Timed Up and Go,TUG)及治疗依从性随访表进行测评。结果干预前,2组的BBS、MoCA、TUG评分差异无统计学意义(P>0.05)。干预6个月后,观察组BBS评分高于对照组[(42.57±5.77)分vs(34.72±8.54)分,P=0.001];观察组MoCA评分为,高于对照组[(24.51±1.25)分vs(21.72±2.96)分,P=0.001];观察组TUG所需时间低于对照组[(18.6±7.80)s vs(25.76±22.67)s,P=0.001];观察组治疗依从性高于对照组(92.71%vs 73.91%,P=0.000)。2组差异均有统计学意义(均P<0.05)。结论对轻度认知障碍患者实施步行—认知双重任务训练,有助于提高患者的平衡功能、认知水平、步行能力及治疗依从性,值得推广。 Objective To explore the effects of walking-cognitive dual task training on improving balance and cognitive ability of patients with mild cognitive impairment.Methods 94 patients with mild cognitive impairment admitted to Third Rehabilitation Hospital of Shanghai City from December 2020 to August 2021 were selected and randomly divided into control group(n=46) and observation group(n=48) by numerical table method.The control group received routine walking training.The observation group added cognitive function training on the basis of routine walking training.The intervention period was 6 months.Before and after intervention,the patients in the two groups were evaluated with Berg Balance Scale(BBS),Montreal Cognitive Assessment(MoCA),Timed Up and Go(TUG) and treatment compliance follow-up table.Results Before intervention,there was no significant difference in scores of BBS,MoCA and TUG between the two groups(P>0.05).After 6 months of intervention,the BBS score of the observation group was higher than that of the control group [(42.57±5.77) vs(34.72±8.54),P=0.001],MoCA score was higher than that of the control group [(24.51±1.25) vs(21.72±2.96),P=0.001] and TUG time was shorter than that of the control group [(18.6±7.80)'s vs(25.76±22.67) s,P=0.001].The treatment compliance of the observation group was higher than that of the control group(92.71% vs 73.91%,P=0.000).The differences between the two groups were statistically significant(all P<0.05).Conclusion The walk-cognitive dual task training for patients with mild cognitive impairment is helpful to improve their balance function,cognitive level,walking ability and treatment compliance,which is worth promoting.
